The Service Coordinator, CBIS will facilitate linkages between and among institutional and community partners in KFLA or HPE or LLG with the intent of increasing the capacity of, and access to services for individuals with Acquired Brain Injury (ABI).

The Service Coordinator will provide leadership and coordination in the provision of client interventions to enable and support participation by people with brain injuries in life roles that are meaningful and satisfying and will be responsible for service coordination, leadership and supervision with regard to the CBIS clients, staff and program activities in KFLA or HPE or LLG. The Service Coordinator will also be responsible for the management of staff in KFLA, HPE or LLG. Duties of the Service Coordinator will be carried out with clients, their family members, CBIS staff and personnel from other agencies and will occur in the community at large, the clients’ homes, CBIS Assisted Living Program or the office of the Service Coordinator.

Skills And Qualifications

  • Three year community college Diploma in Human Studies or related discipline; Degree preferred
  • Minimum five years’ experience in the provision of community rehabilitation and support services to adults with acquired brain injuries preferred, including demonstration of progressively responsible leadership.
  • A combination of education and experience may be considered.

Position Responsibilities

  • Follows and ensures staff follow the Code of Conduct for Community Brain Injury Services
  • Ensures front line management of the assigned staff (in KFLA, HPE or LLG) including, recruitment and selection, orientation, training, performance management, staffing and payroll documentation
  • Participates in the development, implementation and evaluation of CBIS staff orientation and training with respect to the skills and attitudes required to provide effective rehabilitation and support to persons with brain injuries and their families
  • Provides supervision and support to Community Rehabilitation Counsellors (CRC) in the development, implementation and monitoring of strategies to assist clients to participate fully and effectively in life roles that the clients have identified as meaningful and satisfying.
  • Collaborates with institutional and community partners in the development of an effective service system for clients and families affected by brain injury in KFLA, HPE or LLG
  • Develops and maintains a current inventory of all ABI services, publicly and privately funded, within KFLA, HPE, or LLG
  • Actively participates in relevant committees or networks in KFLA, HPE or LLG that support the development of an effective service system for clients and families affected by ABI
  • Participates in decision making about resource allocation for services in KFLA, HPE or LLG
  • Makes timely contact with referred individuals and their families and provides information to new clients
  • Provides service navigation by processing all referrals for services in KFLA, HPE or LLG including review of service needs with client and significant other, making referrals to required services, consulting with other providers as required and completing standardized intake interview and referral documents.
  • Provides 1:1 education to clients as appropriate.
  • Attends and facilitates client reviews, family case conferences, and planning meetings as part of a team, and according to roles identified by clients to facilitate progress toward client role achievement
  • Participates in on-call back-up system in KFLA, HPE, or LLG on a rotating schedule
  • Reviews all program specific documentation and directs to Psychologist and/or Program Manager as required.
  • Assists with the development of reports as required (i.e. Quality Indicators, Service reporting to Ontario Health, etc.)
