The Case Manager supports a specialized client group: people with disabilities, newcomers, Indigenous, and young people with higher needs succeed in retaining sustainable employment. By following a collaborative and client-centered process, this position supports the timely access to the right services and supports to help a member achieve their employment goals and monitors their progress in achieving those goals. The role is responsible for recruiting employers to utilize the employment program, building and maintaining relationships with them and working together to develop employment opportunities suited to fit the needs of both the member and the employer. The Case Manager will actively monitor the employer-member relationship to ensure positive outcomes by conducting ongoing career development and support on the job, assisting clients to meet their job requirements when necessary, and actively and immediately supporting those at risk of losing their job.

In this role, you will:

  • Contribute to monthly individual and team Key Performance Indicators (KPIs) while managing other tasks through prioritization and effective time management.
  • Work with a specialized client group (youth) to ensure they succeed in retaining sustainable employment.
  • Utilize consistent intake tool and approach to evaluate individuals’ Employment Service and Life Stabilization service needs.
  • Actively recruit employers and seek out job placement opportunities, including identifying and contacting new prospective employers in industries/labor market that members have identified as an area of interest and conducts ongoing research of and outreach to employers, community resources and programs.
  • Develop strategies and timelines for individual members for achieving goals while problem-solving when obstacles or challenges occur.
  • Act as a liaison between member and employer to ensure the placement goes smoothly.
  • Develop and monitor training plan and agreements with client and employer; provide ongoing career development and support, on the job, to ensure client and employer are getting the support they need for success.
  • Intervene with immediately support for those at risk of losing their job.
  • Spend time on the job with the client providing training at their worksite, and assists with meeting the job performance requirements as needed to support client in retaining their employment. This may include helping them develop appropriate work habits, behaviours and attitudes for the workplace.
  • Work to understands member’s skills, abilities and areas of interest to help facilitate placements, eg. by reviewing member resumes.
  • Facilitate employer related workshops to groups of members.
  • Utilize electronic tracking system to record and report on the services being offered to individuals.
  • Prepare case management and case note documentation and handle confidential data; ensure quality audit measures are achieved in accordance with the YMCA and funder’s standards.
  • Input, maintain and update member information and outcomes in a timely manner into database.
  • Provide documentation and handle confidential data in accordance with the YMCA and funder’s standards. Participate in the compilation of internal and external reports and statistical data. Quality assurance with respect to assigned tasks to ensure compliance with program standards.
  • Perform other duties as assigned

You bring:

  • Post Secondary Degree in Business, Marketing or related field or equivalent training/experience
  • Minimum two years’ experience in counselling, file management, group facilitation, or job coaching is preferred
  • Demonstrated ability to thrive in a results-oriented role and an ability to motivate and inspire self and others
  • Access to a motor vehicle is required for this position, as extensive travel within the region is required
  • Ability to provide excellent service delivery
  • Well-developed interpersonal, and relationship building skills; ability to establish rapport and excellent communication with members, staff and volunteers
  • Excellent written communication skills
  • Excellent understanding of diversity, inclusion and equity including the ability to identify and address barriers to employment, work effectively with diverse stakeholders, and integrate equity into program design
  • Flexibility regarding assigned work hours and location
