Salary Range: $95,000 - $105,000 plus additional earnings based on client revenue and vehicle allowance.

As the new leader of one of Bayshore’s local Neighborhood Health Teams (NHTs), you will collaborate with a regional team to oversee a range of in-home care services, manage client service programs, and oversee all team operations.

This role is perfect for a nurse professional who is looking for work/life balance and flexibility with their schedule while leading a team of healthcare professionals, and increasing the local access to healthcare.

Powered by Bayshore HealthCare, the successful candidate will join a team focused on promoting well-being and freedom by working closely with each family to help advise, coordinate, and deliver a wide range of services to keep seniors at home as long as possible. This is an active community position that involves a high level of hands-on care, community outreach, and building partnerships with clients and caregivers in the community.

Duties & Responsibilities

The responsibilities of this role include, but are not limited to:

  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals, and continue to resolve client’s challenges by offering services, products, advice, and community resources.
  • Build and sustain strategic and collaborative relationships with current, new and prospective partners and community organizations.
  • Direct and monitor the delivery of all in-home services and daily functions of field staff providing access to clinical advice, support, and resources.
  • Assist with the delivery of in-home services as the on-call nurse, as needed.
  • Lead the sales process, business development, client acquisition strategy and marketing activities for your Neighborhood Health Team (NHT) by assessing community needs and emerging opportunities.
  • Hire and train all NHT staff members, including training and orientation on all services, products, and pricing; supervise the day-to-day performance of the team; ensure exceptional interactions with prospective and existing clients; and provide ongoing coaching and performance management.

Candidates Must Have

  • A current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) registration with British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of five (5) years of recent experience, preferably in a community setting, with at least two (2) years managing others.
  • Current and valid CPR certification.
  • Experience in community work and working with community partners in a collaborative way.
  • Experience with business development and in engaging with municipal and provincial groups is strongly preferred.
  • A reliable vehicle and willingness to travel to visit clients within the community (roughly 50% in the field, vehicle allowance is provided).
  • A solid knowledge of the principles, practices, and methods of program development, implementation, and evaluation.
