Your Opportunity:

This Social Worker II position works out of the Unattached Patient Clinic in Lethbridge and provides a wide range of Social Work services to patients of the Clinic on a referral basis. The Social Worker II provides bio-psychosocial assessment, resource navigation, crisis intervention, education, supportive counselling, advocacy, and consultation as part of their daily work. The Social Worker II has excellent working knowledge of and assists patients to access identified community resources (AISH, Income Support, Prescription coverage, Pensions, Alberta Seniors Benefits) and uses their knowledge of various legislative acts that impact health services (Personal Directives Act, AGTA, Mental Health Act) to secure needed supports. The Social Worker makes appropriate referrals to services that are both internal and external to the healthcare setting and works collaboratively with community agencies/programs for the benefit of Clinic patients and their families. The Social Worker II must apply critical thinking and complex problem solving to serve the needs of vulnerable patients on a day-to-day basis.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum 2 years of professional Social Work experience. This position requires a high degree of independence, critical thinking, and an ability to prioritize problem solving with a view to brief interventions that support brief episodic care delivery. Knowledge of and familiarity with computer applications such as Microsoft Office, Outlook, Teams, Meditech, and/or Connect Care is highly recommended. Previous experience working in the health care system is an asset.

Preferred Qualifications:

Experience working in an interdisciplinary team. Excellent verbal and written communication skills.
