We are searching for a Permanent Full Time Resident and Family Services Coordinator to join our Dartmouth Enhanced Care Team.

About the Opportunity

· Prepare reports, records, statistics and other data as needed or requested.

· Participates as member of the interdisciplinary team in team, client and family meetings.

· Collaborates with clients and family/friends to meet their program expectations and needs.

· Conduct tours of the facilities and interviews prospective clients and/or families through home, hospital and/or office visits to obtain pre-admission information.

· Coordinates admission, transfer and discharge of clients in coordination with the interdisciplinary team.

· Prepares funding requests and reports relating to client needs; maintains client record as required.

· Promotes safety culture (or act as a safety role model) by taking every reasonable precaution in the circumstances to ensure the health and safety of employees and all others in the workplace.

· Ensures client safety by following and applying all standard operational procedures.

About You

· Currently a Licenced Practical Nurse or Registered Nurse actively registered with Nova Scotia College of Nursing

· CPR certified

· Previous experience working in long term care or with seniors.

· Knowledge of the principles of human behaviour and ability to apply program solving skills.

· Strong computer literacy skills.

· Ability to communicate effectively and efficiently (both verbally and in writing).

· Knowledge of available community program and supports.

· Ability to establish and maintain effective, cooperative working relationships with staff, families, clients and other stakeholders.
