POSITION TITLE: Intake and Assessment Coordinator

CLASSIFICATION: MGO-06

SALARY RANGE: Starting at $66,300.00 Per Annum

POSITION TYPE: Full Time, Temporary (Until March 31, 2026, with the possibility of extension)

LOCATION: Saskatoon

ACCOUNTABLE TO: Homelessness Manager

Brief Overview

Under the direction of the Homelessness Manager, the CA Intake and Assessment Coordinator will work together with the Coordinated Access Supervisor and team to coordinate and perform intakes and assessments for people experiencing housing insecurities. This role works directly with the vulnerable population and requires an understanding of housing insecurities and homelessness in Saskatchewan.

Key Responsibilities

  • Conduct intakes, assessments and referrals for people accessing Coordinated Access.
  • Assess and process prospective clients using program assessment measures, respecting client’s dignity, and confidentiality.
  • Monitor, manage and report intake and assessment status.
  • Collaborate with Reaching Home subprojects to ensure people requiring homes are referred to the appropriate organization and housing option.
  • Empower clients by educating them about their rights and responsibilities.
  • Manage the By-Names List through a data-management system.
  • Develop and maintain an inventory list of available housing options.
  • Develop effective and collaborative working relationships and partnerships with community agencies and stakeholders.
  • Stay current with evolving practices and emerging issues that have the potential to affect clients experiencing housing insecurities and homelessness.
  • As a housing team member, contribute to the creation and continued improvement of data collection processes and strategies.
  • Contribute to the creation and continued improvement of strategies and policies to advance the goals of Coordinated Access.
  • Other duties as assigned by the Homelessness Manager or the Director of Housing and Homelessness.

Knowledge And Skills

  • Extensive knowledge of available community resources.
  • Knowledge implementing trauma informed care and harm reductive practices.
  • Knowledge in mental health and addiction services.
  • Must be able to build relationships with community members and community agencies.
  • Ability to communicate in Indigenous languages is considered an asset.
  • Excellent interpersonal skills.
  • Effective verbal and written communication skills.
  • Understanding of the unique challenges and needs of vulnerable citizens.
  • Understanding of the factors which contribute to homelessness or risk of homelessness.
  • Ability to demonstrate a compassionate, solution-focused, and client-centered approach.
  • Strong organizational, analytical, problem-solving, and reporting skills.
  • Proficient in Microsoft Office (Word, Excel, SharePoint).
  • Experience in data management.
  • Ability to remain calm and address clients with patience and understanding.
  • Creative problem-solving, good organizational, administrative, and multi-tasking abilities.
  • Understanding of legislation, regulations, standards, and policies.

Qualifications

  • Bachelor’s degree in social work or other relevant field of study. An equivalent level of education and experience will be considered.
  • A minimum of 2 years’ experience in Human Services working with the vulnerable population.
  • 2 years of Data entry and reporting is preferred.

Additional Requirements

  • Must undergo a Criminal Record Check with the vulnerable sector
  • Valid Saskatchewan driver’s license and access reliable transportation
