Job Summary

This position works within the Nurse Practitioner's (NP) scope of practice in the following category of practice: Family/All Ages or Adult. The NP position is an advanced practice role providing care to clients and families in an out-patient environment. Within the
Geriatric Division of the
Rehabilitation, Geriatric and Palliative Care Program the nurse practitioner is an integral member of a specialized Geriatrics interdisciplinary team supporting frail older adults and their caregivers. This position will involve work within the Home Dementia Program, a component of the Geriatric Division, established to support individuals with moderate to severe dementia, their caregivers and their primary care network, to live safely at home, to reduce/eliminate unnecessary emergency room visits, and avoid premature placement in long term care. In this role the Nurse Practitioner will complete a variety of assessments related to the care of the older adult including, but not limited to, Comprehensive Geriatric Assessments (CGA) and the Clinical Frailty Scale (CFS) and will support/implement strategies to maintain and enhance independence, while preventing and managing common geriatric syndromes, such as, but not limited to, falls and polypharmacy

The Nurse Practitioner will be required to select approaches of care and technology based on the clients' needs and tailored to the client's social, economic and cultural resources.

The Nurse Practitioner functions in the role of practitioner, consultant, educator, leader and researcher in close affiliation with the other members of the geriatric team. The Nurse Practitioner integrates advanced knowledge and experience in delivering safe, effective, quality care to older clients; advocates within the health care delivery system on behalf of clients; liaises with the health care delivery system and community partners to optimize appropriate care; assists in the development, promotion and implementation of best practice in geriatric medicine and specifically within the category of dementia care; interprets and communicate results of investigations to clients, families and primary care providers when appropriate; practices prescriptive authority within the scope of practice for the Nurse Practitioner; and practices with autonomy and collaboration to perform other related duties as required.

The Nurse Practitioner will have frequent and ongoing communication with the lead geriatricians and physicians within the program as well as other members of the interdisciplinary team. The Nurse Practitioner will report to the Geriatrics Division manager within the Rehabilitation, Geriatric and Palliative Care Program. Unit/case assignment will be determined, and is subject to change, based on operational requirements.

Physical effort also includes regular sitting to conduct assessments, home visits and perform work on the computer; regularly stands or walks when assessing clients.
When performing procedures, regularly uses fine finger/precision work and hand tools (i.e. medical instruments, stethoscope, computer mouse, etc.) requiring accurate control and steadiness.
Visual concentration is required when performing physical assessment and procedures, completing charts/reports, observing clients and completing work-related correspondence on the computer.
May be required to stands/walks and work in awkward or cramped positions to complete assessments/procedures.
Constantly uses fine finger/precision, auditory, sensory and visual concentration to provide and conduct risk assessments, observe/monitor clients, complete documents, follow directions, administer medications, etc.
May be subject to time pressures/deadlines.

Job Qualifications
Current registration with the College of Registered Nurses of NL and Licensed (or eligible for licensure) as a Nurse Practitioner (Adult or Family all Ages) with the CRNNL.
Current BCLS Certification is required.

A minimum of five years of clinical experience, of which at least two years has been working
with the geriatric population in any clinical setting is required.

Advanced and current knowledge in comprehensive geriatric assessments, dementia assessments and proficient knowledge in differential diagnosis of dementia, delirium and depression.
Demonstrated strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management and teamwork.
Demonstrated strong communication, problem solving, decision-making, and teaching skills.
Demonstrated progressive clinical leadership skills.

Demonstrated organizational and time management skills.
Demonstrated ability to function autonomously in an advanced practice role.
Demonstrated ability to support and participate in research to improve practice and geriatric care outcomes.
Demonstrated skills in computer, electronic charting and database management is required.
A satisfactory record of work performance and attendance is required.

Completion of an approved/recognized Nurse Practitioner program is required.

A valid Class 5 Driver's License and use of private vehicle is required.
