Sunnybrook Health Sciences Centre is recruiting a full time Nurse Practitioner to be a core member of the addictions team. This role will support Addictions Services at Sunnybrook through the Rapid Access to Addictions Medicine Clinic (RAAM clinic) and the Addictions Consult Service. It is an exciting opportunity for someone with addictions experience who is interested in direct clinical care, as well as supporting the development and implementation of substance use pathways and integrated care. The NP works collaboratively with a multi-disciplinary team in addictions, as well as across the hospital.

Summary of Duties

RAAM Clinic

  • Sees clients with addictions and concurrent disorders at the clinic with or without scheduled appointments
  • Works within the scope of practice of the RN (EC), performs a comprehensive health or symptom directed assessment of the client and synthesizes data from multiple sources to make diagnosis.
  • Ensures that standards of patient care are maintained according to the Standards of Nursing Practice, the Standards of Practice for Nurses in the Extended Class, as well as the Regulated Health Professionals Act and the Nurses' Act.
  • Determines the need for and orders appropriate diagnostic tests and procedures; interprets the results of screening and diagnostic laboratory tests, within the scope of practice.
  • Prescribes medications based on assessments, diagnostic reasoning and test results.
  • Working in collaboration within the inter-professional team, provides advanced nursing care and treatment (including pharmacological complementary therapies and/or counselling interventions for health problem).
  • Maintains accurate and timely records. Write reports, support letters, client files, outcome measurement tools and chart summaries for transfer and/or legal purposes, as well as other related patient and workload measurement documents, as required.
  • Provide clinical leadership for community RAAM clinics.

Addictions Consult Service (ACS)

  • Participates in the delivery of the consult service across hospital units, as well as within key community partner settings (e.g. Bayview, St. John’s, Pine Villa)
  • Provides assessment, treatment recommendations, and support for staff in the implementation of recommendations as required.
  • Is able to triage and prioritize urgent vs. non-urgent consults.

Teaching

  • Supports RAAM and ACS teaching priorities including didactic lessons on identified topics, and supervision of residents and other learners as appropriate.

Program Development

  • Works in collaboration with manager, physicians, team members and hospital and community partners to continue to develop;
    • pathways that support increased access to addictions expertise for patients across the hospital,
    • skills building activities to increase comfort and competency of hospital staff to provide care for patients with substance use disorders,
    • outpatient addictions services that meet need the needs of the North Toronto Health Team population.
  • Participates in committees and working groups as requested by Manager.
Qualifications/Skills

  • Current Extended Class registration with the College of Nurses of Ontario
  • Completed College of Nurses of Ontario approved Controlled Substances Education Program.
  • Knowledge and experience with people experiencing substance misuse as well as illnesses and problems related to such misuse.
  • Sound knowledge of the Mental Health Act, Health Care Consent Act, and other related legislation in Ontario
  • Sound knowledge of psychotropic medications including usage and potential side effects
  • Advanced knowledge of pharmacology in selecting, prescribing, monitoring and dispensing drugs
  • Current B.C.L.S.
  • Program planning and evaluation skills an asset
  • Demonstrated professional leadership and ability to be innovative and creative environment
  • Strong interpersonal and communication skills
