Job Summary

The Clinical Lead – Allied Health, Bundled Care Programs, is responsible for the daily operations. Responsibilities include planning and executing activities related to service delivery and performance management of allied health staff. Provides effective leadership and clinical support to allied health staff. In conjunction with the Senior Clinical Manager, Bundled Care Programs, monitors contractual performance through analysis, tracking and trending and implements strategies for performance improvement.

Primary Responsibilities

  • Support the Bundled Care Programs by ensuring compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
  • Complete patient home visits in instances of training, performance evaluation, employee absences and vacation
  • Attend meetings as necessary for case conferences, providing updates to family, etc.
  • Identifies, implements and sustains best practices and procedures.
  • Plans and coordinates activities to achieve department performance standards.
  • Develops, implements and evaluates training and education activities
  • Provides professional expertise related to clinical treatment, within scope of practice.
  • Facilitates and maintains effective communications strategies to ensure quality client care and foster positive relationships, both internal and external to the agency.
  • Organizes and conducts meetings.
  • Communicates relevant information regarding practice issues
  • Effectively manages client and customer issues and concerns.
  • Assists in the planning, implementation and evaluation of risk management strategies.
  • Maintains appropriate equipment inventories and materials and maintenance.
  • Implements and ensures compliance with records information management protocols with therapy staff
  • Recruitment, hiring and orientation of new staff to meet program needs
  • Identifies training needs and facilitates acquisition of skill sets to meet program needs.
  • Conducts regular professional reviews for therapy staff
  • Participates in community care activities utilizing information for department planning, development and evaluation.
  • Monitors and analyzes the intake process, priority codes and refusals and implements processes to improve rates
  • Ensures all aspects of the client assessment process are completed.
  • Monitors and analyzes the achievement of client care goals and outcomes
  • Ensures that services are delivered according to best practices and established procedures and protocols.
  • Work safely at all times according to Bayshore standards.
  • Must be available to work flexible hours which may include evenings and weekends
  • Some travel may be required, must have own car with driver's license and valid insurance
  • Complete other tasks as requested.

Minimun Qualifications

  • Exemplifies characteristics of Bayshore Health Care: compassion, excellence and reliability.
  • In good standing with appropriate college/governing body in the province of Ontario
  • Graduate of an accredited and approved program
  • 5+ experience in health care, preferably home health care & hospital
  • 3-5 years of community experience
  • A Regulated Health Professional in good standing with their respective college (Physiotherapy or Occupational Therapy )
  • Completed Continuing Education Requirements (CEUs) as required by regulation.
  • Demonstrated record of building and maintaining referral relationships.
  • Demonstrated knowledge of the Canadian health care system including; OHIP, insurance & private pay.
  • Knowledge of medical terminology, patient care best practices and infection prevention and control
  • Previous discharge planning experience.
  • Ability to provide excellent customer service.
  • Demonstrated ability to work well in a team environment
  • Demonstrated leadership skills
  • Working knowledge of business office technologies: PC/laptop with Microsoft Office applications, smart phone, website/internet, email.
  • Ability to read, write and effectively communicate in English.
  • Must be available to work evenings and weekends as needed

STANDARDS OF PERFORMANCE

The Clinical Lead- Allied Health must demonstrate ongoing competency in completing all expected duties and responsibilities as detailed in this job description as well as specific goals and objectives that are agreed to.

PERFORMANCE REVIEW

Performance will be reviewed on an ongoing basis with a formal review at the end of the three month probationary period and subsequently, on at least an annual basis. Input will be sought from the employee, their Supervisor, employees at the Unit and in addition, will be based on ongoing client feedback and the extent to which performance meets expectations.
