Bloom Care Solutions is looking for a Part-Time Occupational Therapist in Kitchener/ Waterloo/ Cambridge/ Brantford and surrounding areas.

Requirements/Qualifications

• A regulated health professional Occupational Therapist (OT) with a minimum 2 years experience
• Registered and in good standing with the College of Occupational Therapists of Ontario
• ADP Authorizer is preferred but not required
• Great communication skills - must be able to effectively communicate and think critically (both written and verbal skills)
• Valid drivers license and willingness to travel in Kitchener/Waterloo/Cambridge/Brantford area
• Strong time management skills, confidentiality and professionalism
• Excellent communicator, able to foster relationships with Home Care Staff and clients
  
  
  

Responsibilities

• Evaluate the effectiveness of treatment plans and modify them accordingly and complete assessments in a community setting
• Complete the documentation and/or charting to ensure excellent communication in a timely manner
• Identify the need for any beneficial/assistive device for the patient - support with the coordination of the delivery and installation of these devices
• Collaborate with other team members and health care professionals as needed
• Ensure that all patient’s needs are met
