Social Worker (BSW) - Transitional Rehabilitation Housing Program 2 (TRHP 2) Full Time – Permanent
THE ROLE AND OPPURTUNITY
The Canadian Mental Health Association - Toronto Branch is looking for a Social Worker (BSW) for the TRHP 2 program.TRHP 2 is partnered with CAMH’s Complex Care and Recovery Services and is a site-based program located at 30 White Squirrel Way,on the grounds of CAMH’s Queen Street Site. All clients have been found Not Criminally Responsible (NCR) and are under the supervision of the Ontario Review Board (ORB). This position is part of an interdisciplinary team within the TRHP 2 program, an initiative designed to facilitate the placement and support of forensic inpatients into transitional housing. The focus is on rehabilitation and community reintegration with individuals regaining autonomy and rebuilding independent living skills. It provides a unique discharge pathway from hospital to permanent housing. The successful candidate will work Monday-Friday, 8am-4pm, to provide evidence-based programing, contribute to program development/evaluation, and create a collaborative therapeutic milieu.

Responsibilities
Reporting to the Manager of Forensic Services and the Clinical Lead – TRHP 2, the successful candidate will work with the interdisciplinary team to support their clients. They will be required to participate in team meetings and provide regular updates to the manager and lead. They will also act as a liaison between CMHA and CAMH, as well as other community service providers. The aim of this role is for the successful candidate to assist in the development of a service care plan for clients as well as linking them to appropriate community services. This will involve assessment, intake and collaborative strengths-based service care planning, identifying rehabilitation goals, and risk management strategies . Along with orientation to the program, agency, and community, and supporting clients to understand and adhere to the condition of their ORB dispositions.
There is a requirement of providing on-site and community-based services and interventions, including group facilitation, one to one therapeutic support, support with ADLs and maintaining meaningful and structured daytime activities, social recreational programming, psycho-education, and structured relapse prevention. This role will involve risk assessment/management, crisis prevention/intervention, monitoring, evaluation and follow-up. The successful candidate will also work to maintain trusting, respectful and collaborative working relationships with clients and colleagues. There may also be coordination of site maintenance, of the TRHP 2 site, where the position is based out of.
Skills You Bring
CMHA Toronto’s ideal candidate will have a Bachelors Degree in Social Work. Registration, in good standing, with a professional college is considered an asset. The candidate will bring a minimum of three (3) years of experience in direct service delivery to adults with serious and complex mental health issues, including those with concurrent disorders and those with criminal justice and/or forensic involvement.
The candidate will have skills in Cognitive Behavioural Therapy (CBT), Dialectical Behaviour Therapy (DBT), Motivational Interviewing (MI) and supportive counselling. Both 1:1 service provision and group facilitation are a requirement. The ideal candidate will bring knowledge of psychosocial rehabilitation, recovery principles, therapeutic treatment modalities, supportive housing, forensic mental health, the criminal justice system and concurrent disorders.
The candidate will bring excellent organizational, interpersonal, communication, decision making and crisis intervention skills. They will have the ability to work with diverse communities and demonstrate an anti-oppression approach to practice. The candidate will also have the ability to work independently as well as within a dynamic team environment. The candidate will have proficiency in Microsoft applications including Office 365.
