The Program

The Specialized U13 and Mountainside Programs are trauma-informed, relationship-based, Therapeutic Campus-Based Care programs serving children and youth. We incorporate the Neurosequential Model of Therapeutics (NMT), an evidence-based practice developed by Bruce D. Perry, M.D., Ph.D., into our therapeutic milieu and treatment plans.

The Position

We are looking for a Full-time Family and Connection Facilitator.

The Family and Connection Facilitator will serve a key role on the team, ensuring family engagement in the treatment process and supporting the development and implementation of aftercare services. This is an exciting role for a skilled professional who is a team player, understands the campus-based treatment environment and wraparound approach, can provide guidance on effective caregiving and has experience working with families (including indigenous families). Hull works through a trauma informed lens, using relationships and regulation strategies to foster resilience and healing. The Family and Connection Facilitator will work with a community of practice in the areas of family work, wraparound, and cultural services.

Duties And Responsibilities

  • Gather key information regarding the young person’s family and extended support system.
  • Assess the young person’s and family’s clinical needs and strengths; communicate this information with relevant parties, including natural support systems, staff and external professionals throughout the assessment and treatment process.
  • Pro-actively coordinate both face-to-face and telephone contact between support systems while the young person resides in the program.
  • Continually assess relevant family and extended support system information; assess, identify and facilitate intervention and family support opportunities.
  • Meet with families to begin parenting and in-home work to promote greater family cohesion, effective communication, solution-focused problem-solving and a shared commitment to change.
  • Assess overall family functioning and recommend additional services as required.
  • Develop a solid bridge between the treatment program and the next identified placement; ensure that key information regarding supports, strategies, strengths and therapeutic gains are effectively communicated.
  • Coordinate and provide flexible aftercare services to ensure that transition planning and post-discharge supports continue to be in place.
  • Flexibility in scheduling (evenings and weekends) is required in order to facilitate family visits.
  • Building, supporting and coordinating natural supports and overall connections for all children and youth
  • Involvement in overall therapeutic milieu may include working on the floor with the children at times to support the team.

Skills And Qualifications

  • Degree or Diploma in Human Services or Social Sciences.
  • Experience in Therapeutic Campus-Based Care treatment (preferred).
  • Ability to engage children, youth and families in the treatment process.
  • High level of personal competence in the formation and maintenance of caring, therapeutic relationships.
  • Strong relational and engagement skills, including proficient and professional communication with youth and families.
  • Ability to use a family-centered, trauma-informed, and strength-based approach towards service delivery.
  • Relevant experience working with youth and their families and knowledge of crisis intervention, communication skills and family education.
  • Experience working with and incorporating indigenous and cultural perspectives into service plans.
  • Strong interpersonal, and networking skills.
  • Ability to work independently and as part of a team; experience working on a multidisciplinary team.
  • Demonstrated interest, commitment, consistency, and a high level of personal maturity and responsibility.
  • Flexibility and adaptability.
  • Clear Police Check including Vulnerable Sector Check and Child Intervention Record Check.
  • Reliable transportation, valid driver’s license and ability to drive Agency vehicles.
  • Completion of ASIST, First Aid, Therapeutic Crisis Intervention and Certification with CYCAA would be considered an asset.
