Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Health Care Aide to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. The Health Care Aide provides personal care and support to clients in a community setting. Work is assigned according to an established plan of care, in collaboration with the client, family and members of the multidisciplinary team. The Health Care Aide is supervised by a regulated health professional and may perform assigned restricted activities under specific circumstances. Documentation of pertinent information and care provision is required, as is participation in relevant in-service education. The Health Care Aide provides patient care services as an unregulated worker under the supervision of a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), or Licensed Practical Nurse (LPN). This position will be based out of St. Albert Homecare but will provide support to home care's Suburban/Rural West team, which will include travel to the other home care network offices, as required
Description:
As a Health Care Aide (HCA), you will work as a collaborative member of the multi-disciplinary team providing personal assistance including supporting activities of daily living and support services to patients (clients, residents) who require short-term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this position you will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities assisting nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation and safety needs, assisting with organized activities or programs and observing and reporting on a patient's mood, health, and wellbeing. Working under a regulated health professional you will be assisting in the maintenance of a safe environment for patients, staff and visitors, and maintaining effective communication with patients and health professionals
