As a member of the Geriatric Psychiatry Program, you will use best practices to provide person-centred Social Work services working alongside seniors with complex mental illness and their families. This includes assessment, therapeutic intervention and discharge planning leading to successful transitions in care; in collaboration with the specialized interprofessional clinical team, client, and families.
Duties:
- Provide individual, family and group therapy following best practice standards, consistent with the Ontario College of Social Workers and Social Service Workers.
- Complete risk assessment and crisis intervention plans.
- Engage client, family and caregivers in the clinical interventions and risk reduction strategies with the interdisciplinary team.
- Advocate on behalf of clients, families and caregivers for appropriate services and resources.
- Collaborate, consult and liaise with team members and community resources.
- Provide timely and comprehensive written assessments, treatment and discharge reports.
- Demonstrate thorough knowledge of the theories and practices of a professional social worker.
- Maintain workload measurement and data collection as required (i.e. RAI).
- Participate in the teaching, education and supervision of Social Work students.
- Complete and maintain all requirements for registration with OCSWSSW.
- Attend Social Work clinical peer consultation groups, and participate in peer consultation processes.
- Attend Social Work meetings and participate in discipline-specific committees.
- Keep current of hospital policies and procedures and applicable legislation.
- Keep up to date regarding professional standards by reviewing current literature, attending education events, networking with colleagues, etc.
- Provide educational presentations to community groups.
- Flexibility in work schedule; day and evening hours as required.
- Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the Royal.
- Ensure a work environment that is conducive to the Royal’s Harassment-Free Workplace Policy.
Qualifications:
- Master’s degree in Social Work.
- Minimum 2 – 3 years clinical Social Work experience in a mental health setting.
- Member in good standing with the Ontario College of Social Workers and Social Service Workers.
- Demonstrated ability to work with an interdisciplinary team.
- Person-centred approach.
- Experience in group work.
- Conflict resolution skills.
- Knowledge of applicable legislated acts (e.g. MHA, PHIPA, The Health Care Consent Act).
- Ability to manage and determine priorities with a diverse case load.
- Strong advocacy skills.
- Ability to work in an automated work environment.
- Proficiency with latest versions of Windows, Microsoft Word, Excel, Internet Explorer, and Outlook.
- High level of knowledge, skill, and confidence in using current Meditech system for clinical documentation and navigation.
- Discharge planning experience with Seniors, knowledge of related community resources and access to those resources.
- Ability to conduct group work with persons with varying levels of cognitive and physical abilities is required.
- Sound understanding of the effects of physical aging and disease, dementia and responsive behaviours as well as an understanding of areas of vulnerability for this population.
- Valid driver’s licence and access to reliable vehicle is preferred.
- English level A- is mandatory in oral comprehension, expression, reading, and writing. Bilingual (French/English) is considered an asset.
Salary Range:
44.67 to 57.90 per hour
