- Primary Care Services: Providing comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and the elderly.
- Preventive Care: Conducting routine check-ups, health screenings, and immunizations to prevent illnesses and promote overall health and well-being.
- Diagnosis and Treatment: Diagnosing and treating common acute illnesses, such as respiratory infections, urinary tract infections, and minor injuries, as well as chronic conditions like diabetes, hypertension, thyroid disorder and asthma.
- Chronic Disease Management: Managing chronic diseases through ongoing monitoring, medication management, lifestyle counseling, and referrals to specialists when needed.
- Health Promotion and Education: Providing health education and counseling to patients on topics such as nutrition, exercise, smoking cessation, and stress management to promote healthy lifestyle choices.
- Mental Health: Promoting mental health and preventing substance use disorder by providing initial assessment, treatment and initiating referrals as appropriate.
- Referrals and Coordination of Care: Referring patients to specialists, diagnostic tests, and other healthcare services as necessary, and coordinating care among multiple providers to ensure continuity and quality of care.
- Prescription Management: Prescribing medications, reviewing medication regimens, and monitoring for potential drug interactions or adverse effects.
- Minor Procedures: Performing minor surgical procedures, such as removing staples, moles, warts or skin lesions, ear syringing and administering injections.
- Telemedicine Consultations: Providing virtual consultations and follow-up care through telemedicine platforms, especially in situations where in-person visits are not feasible or necessary.
- Electronic Health Record (EHR) Documentation: Maintaining accurate and up-to-date electronic health records of patient encounters, including medical history, physical examinations, diagnostic test results, treatment plans, and follow-up care.
