- Have a master’s or bachelor’s Degree in a Regulated Rehabilitation Health profession
- Possess a current registration with the applicable regulatory College in the province of Ontario
- Have 2 years of supervisory experience and/or experience within home health service delivery
- Be able to effectively establish and build rapport with clients and therapy service providers
- Computer skills – Proficiency with Microsoft Office 365, Teams, and the ability to use and utilize electronic charting
- Possess a valid Ontario driver's license with access to a reliable vehicle with insurance
- French fluency both verbal and written is considered an asset
Promote a positive Client and Family Experience
- Ensure the service provider has the skills, competencies and experience to meet the client’s service plan.
- Respond to client complaints striving to resolve when able and escalating as appropriate.
- Identify opportunities to improve service delivery, engaging appropriate team members to drive effective and efficient change as required.
- Ensure client experience is satisfactory, review trends from enhanced quality checks, share and communicate with discipline Practice Lead (PL) team members and ensure follow up to identified clients and families.
- Lead by example; promote service delivery excellence through effective onboarding, mentoring/ coaching, and managing performance of service providers.
- Complete formal reviews with new hires at the 30-, 60- and 90-day check-ins.
- Ensure all team members are aligned to the Key Accountabilities of their role and are meeting quality & performance expectations.
- Actively participate in maximizing staff utilization, retention and engagement.
- Develop and update clinical resources related to service delivery for all therapy disciplines in collaboration with Practice Leads (PLs) and Care Manager.
- Monitor and supervise specialized therapy program implementation. Create, expand, implement, and evaluate program development and outcome measures. Includes training, and developing resources, electronic health record forms and processes to standardize care. Work to scale and spread programs to other geographies and disciplines.
- Support and guide multi-disciplinary student placements, when applicable.
- Supervise a team of Practice Leads (PLs) from each therapy discipline including organizing and facilitating meetings, providing clinical guidance, responding to complaints and problem solving.
- Facilitate team/discipline specific meetings, huddles, and other structured communication opportunities.
- Ensure all reporting meets or exceeds all standards and expectations of CBI Health, stakeholders, and the associated regulatory College (as applicable).
- Guide and monitor chart audit process.
- Respond to sudden changes/demands in staffing in a manner which supports a positive client experience. Communicate HR challenges with management and assist with problem solving around capacity limitations.
- Proactively identify risk and work with all stakeholders to mitigate and/or manage risk with particular attention to missed visits and clinical service delivery.
- Ensure a timely and comprehensive response to Incident Reporting, and investigation to address Funder, Client and Service Provider Safety Incidents and Client Complaints.
- Provide input into developing clinical education and training needs and/or opportunities.
- Working Conditions: inclement weather, dealing with escalated situations with client or families, staff or others.
- May be required to travel within the region
- Hybrid: combination of on site and working from home on a weekly basis
- On call rotations and work outside of regular office hours may be required
