As a Centralized Mental Health Intake Worker, you will help children, youth, and families connect with the right mental health and addiction services when they need it. The position involves initial screening of referrals, assessment with children, youth, and families, liaising with referral sources, and assigning cases to teams.

Responsibilities

  • Makes direct contact with children, youth, and families or healthcare providers as needed to collect appropriate information and determine appropriateness for services.
  • Electronic tracking of referrals and patients’ status and related activity within all programs and teams.
  • Respond to urgent care calls from community organizations, school boards, community physicians, and Emergency Departments outside of Ottawa within the Young Minds Partnership catchment area.
  • Respond to requests for information from families about the referral process and status of waiting lists.
  • Determine optimal services within 1Call1Click.ca services, liaise with appropriate teams for case distribution.
  • Monitor services received and waiting lists.
  • Work closely with community agencies and private practitioners.
  • Advise the 1Call1Click.ca Partnership on Intake-related issues.
  • Track status of referrals.

Qualifications

  • Vulnerable Sector Check (Level 3) (Essential)
  • College Diploma as a Child and Youth Counsellor (Essential)
  • Eligibility for certified membership in the Ontario Association of Child and Youth Counsellors (OACYC) (Essential)
  • Minimum three (3) years of related experience (Essential)
  • Extensive knowledge of community resources (Essential)
  • Experience working in a mental health team (Essential)
  • Strong interpersonal and teamwork skills (Essential)
  • Comprehensively use relevant computer technology (Essential)
  • Ensure timely completion of tasks by adjusting priorities (Essential)
  • Effective communication skills, both orally and in writing (Essential)
  • Bilingualism (English and French) (Essential)
