ABOUT THE COMPANY
Main Street Project (MSP) is a community health Centre addressing social determinants of health that lead to addiction, mental health issues, and homelessness. Utilizing housing-first and harm reduction principles, MSP provides a safe, respectful, and accessible environment for those at risk of homelessness. Since 1972, it has offered services such as emergency shelter, withdrawal management, health advocacy, case management, and community outreach.
JOB SUMMARY
The Intake Worker connects with potential and current participants of MSP's Withdrawal Management Program. Responsibilities include assisting individuals in accessing MSP services, connecting them with primary care and mental health supports, and maintaining communication post-treatment for ongoing support.
KEY RESPONSIBILITIES
- Bed Intake Monitoring: Ensure timely placement for withdrawal management service seekers at two locations.
- Client Outreach: Assess needs and provide information about the program, supporting the admission process.
- Post-Treatment Follow Up: Maintain connection and provide resources to participants after treatment.
- Assistance with Transports: Facilitate transportation for participants to treatment facilities.
- Collaborative Treatment Planning: Develop personalized treatment plans with WMS Case Workers.
- Support Indigenous Relations: Encourage cultural programming through Healing and Harm Reduction.
- Professional Relationship Maintenance: Establish clear boundaries and maintain accurate documentation.
- Maintain Records: Ensure continuous, legible records in compliance with established protocols.
- Account for Power Differential: Be mindful of the dynamics between the Intake Worker and participants.
- Planning: Manage incoming and outgoing participants who have connections with MSP.
- Resource Identification: Provide information on community and mental health resources.
- Suicide Ideation Support: Identify and address signs of suicidal thoughts.
- Cultural Safety: Create safe environments for participants.
- Regular Progress Communication: Update participants and MSP staff on progress.
- Data Management: Keep confidential electronic and paper records.
- Crisis Intervention: Respond to crises as needed.
- Community Engagement: Attend relevant community meetings.
QUALIFICATIONS
- Experience with unsheltered populations and case management.
- Documentation and planning skills.
- Experience with Indigenous communities.
- Proficiency in Microsoft Office and service delivery databases.
- Relevant certifications in first aid and mental health.
