We are searching for a caring and competent RPN or Registered Nurse to join our expanding team as a Client Care Coordinator based in our Peterborough, Ontario office.

Salary Range: $68,000 to $75,000 annual salary
Objective: The Client Care Coordinator (CCC) nurse is based in the office and also travels to client's homes regularly. The CCC performs duties related to client care including care consultations with potential clients and family members, client and caregiver introductions, quality assurance visits with existing clients and performance reviews with the care team. The CCC uses a consultative approach to determine each client's needs to provide solutions and create a tailored care plan. The CCC continuously evaluates the plan through ongoing communications and visits to ensure quality care, client satisfaction and retention and opportunities to increase service hours.
Primary Responsibilities:
- Meet with potential clients and family members to discuss their needs and provide care solution options
- Conduct Service Inquiries and Care Consultations as needed in a consultative manner
- Conduct client/caregiver introductions with new clients and with every new PSW/caregiver.
- Create and maintain client care plans and responsible party records documenting all quality assurance meetings.
- Collaborate with Service Coordinator and Recruitment to ensure strong client and caregiver/PSW matches
- Work with clients and their families on the various issues that may arise to ensure they receive timely resolution to issues
Secondary Responsibilities:
- Lead and participate as needed in care team meetings and orientations
- Perform after-hours weekly emergency on-call support as needed
Education/Experience Requirements:
- RPN or RN license in good standing with the College of Nurses of Ontario
- Minimum of three years experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
- Must possess a valid driver's license and be willing to drive to and from client's homes
- Must demonstrate excellent oral and written communication skills and the ability to listen effectively
- Must have the ability to work independently, maintain confidentiality of information and meet deadlines
- Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
- Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
- Proficiency in computer skills and in Excel and Word
