The Canadian Mental Health Association - Champlain East is a community-based agency dedicated to providing community support services to persons living with a mental illness and concurrent disorders. Our Branch has been designated under the French Language Services Act since 1991 and as such, is committed to providing services in both official languages (French and English).

All applicants must be fully vaccinated against COVID-19 in order to be considered for a position with CMHA Champlain East, unless they have an exemption on a medical ground. Upon hiring, proof of vaccination or exemption will be required.

Intensive Case Manager

PURPOSE OF THE POSITION:

To provide community support services to persons with a severe and/or persistent mental illness or concurrent disorder, with the goal of independent living, community integration and the least reliance on the formal mental health system. The service is individualized and portable. The Intensive Case Manager (ICM) is responsive to the clients’ multiple and changing needs and plays a pivotal role in counselling and coordinating required services from across the mental health system as well as other service systems. The ICM fulfills a vital function for clients by working with them to help them realize their personal recovery goals.

QUALIFICATIONS:

Requirements:

  • University degree in the Social Science/Health field with related work experience as a Case Manager working with people with emotional/psychiatric difficulties, or an equivalent combination of education and experience.
  • Extensive knowledge and understanding of mental health disorders and addictions
  • Familiarity with Recovery Philosophy and awareness of the needs of diverse communities is an asset
  • Knowledge of the social services and/or health field, including local resources
  • Well organized and able to work independently and as part of a team.
  • Possess a valid driver’s license and have access to a vehicle
  • Ability to work flexible hours (some evening and weekend hours may be required)

Language:

  • Proficiency in both official languages is a requirement as per the Branch’s French Language Designation Plan

Skills, Knowledge and Abilities:

  • Awareness of local community resources
  • Ability to organize time and prioritize tasks to achieve objectives, demonstrating organization, planning & coordination skills
  • Ability to deal tactfully with a diverse group of people internally and externally
  • Proficiency in Microsoft Office programs, such as Outlook, Word, Excel and PowerPoint
  • Proficiency in virtual platforms such as MS Teams
  • Demonstrates initiative when appropriate and is willing to learn new skills
  • Demonstrated ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with clients/volunteers
  • Engage in reasonable self-care strategies that incorporate balancing work/life responsibilities
  • Excellent problem-solving and decision-making skills

RESPONSIBILITIES OF THE POSITION:

  • Develop individualized Assessment and Plan of Action
  • Provide service in both individual and group format
  • Provide community-based support services as required
  • Link client to appropriate community services and supports
  • Coordinate client services through collaboration with other agencies
  • Monitor, record and evaluate the achievement of client goals
  • Collect data relevant to program evaluation
  • Work flexible hours to meet the needs of the client (evening and weekend hours required)
  • May be assigned a lead role for special projects/programs within the same job classification

SALARY RANGE: $56,766 - $63,937
