SALARY: FROM $39.16/HOUR TO $50.77/HOUR

Qualifications
Certification/Experience/Knowledge:
- Bachelor of Social Work (BSW) required, Master’s in Social Work (MSW) preferred
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and knowledge of associated Code of Ethics and Standards of Practice required
- Recent 3-5 years’ experience in an acute care and/or chronic care hospital setting with coordination of community services required
- Knowledge of Social Work values, ethics, theory and established practices of clinical social work and clinical practice including assessment, treatment and crisis intervention skills required
- In depth knowledge of serious mental illnesses from a bio psychosocial spiritual perspective and experience working with individuals with a serious mental illness within a psychosocial rehabilitation model required
- Knowledge and understanding of the diverse populations and regions served including socioeconomic status and rural challenges required
- Non-Violent Crisis Intervention required (training delivered by Health Centre upon hire)
- Gentle Persuasive approach (GPA) Certificate required
- PIECES certificate an asset
- Thorough knowledge of geriatrics and geriatric related diseases required
- Experience knowledge in Complex Continuing Care as well as palliative care/end of life required
- Knowledge of long term care application/retirement living and relevant knowledge of LTC/Retirement homes in the surrounding area required
- Experience with in reach and outreach services, working with patients prior to and once transferred to discharge destination
Skills/Abilities:
- Demonstrated application of discharge planning principles and resource utilization is required
- Must be able to work with minimum supervision, capable of making timely decisions and exercising tact
- Comprehensive knowledge of relevant legislation including Health Care Consent Act, Substitute Decision Act and Mental Health Act required
- Understanding of the social determinants of health and the social consequences and risk factors associated with serious mental illnesses required
- Must possess high self-direction/self-motivation and be adaptive to change required
- Demonstrated ability to work effectively to meet deadlines required
- Work independently and within a team environment required
- Excellent communication and interpersonal skills required
- Excellent interpersonal, interviewing and counselling skills required
- Case management skills and inter-disciplinary and intersectoral team orientation required
- Experience, skill and punctuality in preparing reports required
- Computer skills required
- Must demonstrate commitment to ensure safety of fellow workers, patients, volunteers, and visitors
- Good attendance and work record required
- Ability to travel between local sites required
- Bilingualism (Advanced level English/French) is an asset
