Summary Of Job Description

NorWest Co-op Community Health Centre is seeking an Intake Worker for our Family Violence and Prevention Programs Team. Reporting to the Director of Family Violence and Prevention Programs, the Intake Worker will play a key role in connecting participants to essential services through comprehensive intake and assessment processes. The role focuses on assessing participants' needs, navigating complex systems of care, managing waitlists, and ensuring appropriate referrals for Family Violence and Prevention Programs in consultation with the Family Violence and Prevention Programs Director. This position is rooted in principles of self-determination, client-informed practice, social justice, strengths-based approaches, harm reduction, and empowerment theories. The Intake Worker will collaborate with a multidisciplinary team while also working independently to provide high-quality, trauma-informed support. Adherence to the social work code of ethics, with a commitment to fostering empowerment, equity, and respect for client autonomy, is required.

Qualifications

  • Bachelor of Social Work (BSW) required.
  • Active registration with an appropriate regulatory body
  • Minimum 2 years of experience in intake or case management within mental health, addictions, gender-based violence, social work, or community programs.
  • Strong understanding of gender-based violence, intimate partner violence, mental health, and substance use disorders.
  • Experience conducting risk assessments and safety planning.
  • Comprehensive knowledge of community-based resources, system navigation, and service coordination.
  • Demonstrated ability to work both independently and collaboratively within an interdisciplinary team.
  • Strong communication, organizational, and documentation skills.
  • Experience working with diverse populations and promoting equity in service delivery.
  • Strong knowledge of trauma-informed care, harm reduction, and strengths-based approaches.
  • Ability to work effectively with diverse populations including indigenous and newcomer communities using culturally safe and anti-oppressive practices.
  • Knowledge of Family Law in Manitoba and experience working within integrated service settings.
  • Commitment to ongoing professional development and training.

Duties

  • Conduct intake assessments to determine client eligibility and needs for Family Violence and Prevention Programs.
  • Provide crisis intervention and safety planning support during initial contacts as needed.
  • Facilitate timely and appropriate referrals to internal and external programs and services.
  • Manage and maintain program waitlists, ensuring regular updates and communication with participants.
  • Coordinate with community and health system partners to streamline service navigation for clients.
  • Document all direct and indirect interactions and service plans using EMR systems per professional and organizational standards.
  • Participate in multidisciplinary consultations to enhance care plans and outcomes.
  • Support participants with applications for Protection Orders or other legal supports as needed.
  • Assist in planning and facilitating group programs, workshops, and other community-based interventions.
  • Monitor program trends and contribute to data collection and reporting for evaluation and funding purposes.
  • Attend team meetings and training sessions to ensure alignment with best practices and program standards.
  • Work flexible hours, including evenings and weekends, as required.
  • Complete other duties as requested, which may include additional training, coverage, and group facilitation.
  • Other duties as assigned.

Additional Requirements

  • Access to a vehicle and a valid driver’s license required.
  • Completion of Child Abuse Registry and Criminal Records Checks required.
  • This role is critical in providing trauma-informed, client-centered support, ensuring access to essential resources and fostering collaboration within the community mental health and family violence sectors.
  • This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
