Job Summary: The Community Health Worker provides services and supports the health promotion and community development initiatives for identified priority populations in the Nepean, Rideau and Osgoode areas in Ottawa.
Job Specific Responsibilities:
- Provides information on, and links clients to, the resources and services of PQCHC, the Nepean, Rideau Osgoode Community Resource Centre (NROCRC) and other community health and social services.
- Works with the Satellite team, in collaboration with NROCRC and other service providers, to assess the needs of priority populations.
- Develops, implements, monitors and evaluates programs/initiatives in collaboration with NROCRC and other service providers to meet identified needs.
- Negotiates and coordinates program delivery sites.
- Acts as a resource to and advocate for identified populations on an individual basis and as groups.
- Assists in organizing support groups, educational workshops and health promotion activities for identified populations.
- Creates and maintains service delivery and planning partnerships with relevant community professionals, agencies and services, including NROCRC.
- Develops and implements community development and outreach strategies to reach priority populations.
- Promotes/advertises and informs the community of programs and services.
- Assists in the recruitment of and supervision of program staff and volunteers.
- Provides monthly activity data (stats).
- Researches and applies for additional program resources (funding proposals) in consultation with the Program Manager.

- Maintaining competence, and where applicable, a professional licence to practice.
- Supporting the Centre’s student and volunteer placement programs.
- Promoting awareness of and participation in Centre activities.
- Contributing to the Centre’s work by participating in meetings and committees.
- Working during both regular and extended hours of operation in locations identified by the Centre.
- Contributing to the Centre’s practices of hiring, orienting and training of staff.
- Contributing to the Centre’s efforts to secure and maximize resources for current and new programs, services, and activities.
- Participating in the Centre’s efforts to enhance its capacity through staff development.
- Working in a manner that supports the Centre’s Occupational Health and Safety Policies and Procedures.
Qualifications:
- Post-secondary degree in education in social sciences/social work and/or equivalent combination of education and experience.
- 3-5 years experience working in a community-based setting.
- Experience working with multicultural communities; families living in low-income; high-risk families.
- Experience in project management.
- Knowledge of/training in group facilitation.
- Ability to work independently and as a member of a team.
- Fluency in English, written and verbal communication skills; other languages an asset.
- Valid driver’s license and daily access to a vehicle an asset (program locations can be widely dispersed with poor access to public transportation).
- Ability to work flexible hours (evenings and weekends expected).
- Computer literacy - Experience with Office 365, Zoom and Sharepoint.
