ROLE PURPOSE

This position will provide outreach support to client(s) referred to Connective by our funders. You will work to enable clients’ adjustment to and success with life in the community.

This is a person-centered role where you will go out into the community to meet clients and develop individualized goal plans which can include accessing community services, housing and employment supports & life skills development. This role will include using your own vehicle to transport clients.

STATUS AND CLASSIFICATION

Unionized. This position is open to applicants of all genders.

HOURS (40 Hours Per Week)

Monday - Friday (0900 - 1700)

REPORTING AND WORKING RELATIONSHIPS

  • Reports and is accountable to the Program Manager or their designate
  • Participates as a member of Connective team to ensure quality and continuity of services to clients
  • Liaises with staff and volunteers of community and government agencies as required
  • Liaises with Connective staff as necessary
  • Accepts clinical direction from the care team
  • Participates as a member of Connective staff team

Key Accountabilities

  • Receives referrals from funders
  • Works with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities
  • Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals
  • Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this plan
  • Ensures the client’s ongoing legal, mental, and physical health issues are addressed in the plan
  • When and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goals
  • Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals
  • On the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services
  • Maintains extensive records and case notes in accord with Community Living British Columbia policies and standards
  • Completes monthly progress reports
  • Participates in staff and resident/staff meetings as instructed by the Program Manager
  • Other duties as subsequently negotiated

WORKING CONDITIONS

  • Functions independently, while remaining a crucial member of the care team
  • Adheres to Connective policies and procedures
  • Is available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules
  • Is a member of CUPE Local 1936-11
  • Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manual

Experience And Skills

  • Graduate with a completed Diploma or Certificate in Social Services or a relevant field with one year of related experience. Relevant fields include Social Work, Criminology & Psychology.
  • Counselling experience in the appropriate field and demonstrate a clear ability to respond well in crisis situations
  • Experience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders
  • Understanding of providing service to people with different learning abilities
  • Extensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvement
  • Experience in assessment, case/care plan development and implementation
  • Demonstrated an ability to deal with potential violence
  • Demonstrated an ability to develop programs tailored to the particular diverse needs of the individual
  • Excellent written and verbal communication skills in English

Requirements

  • Ability to successfully complete a Criminal Records Check
  • Current Emergency First Aid Certificate or Level 1 First Aid
  • BC Class 5 driver’s license
  • Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients
  • Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements

Compensation

As Connective is a unionized environment, the starting wage is $27.54 per hour. Plus benefits enrollment as per the Collective Agreement.

Important: Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
