The Clinical Lead is responsible for coordinating aspects of care for individuals supported by CBI Home Health in our Specialized Community Services in all settings. You will provide care support including aspects of case management, comprehensive current behaviour plans and medical appointment support.

You will provide care support including aspects of case management, comprehensive current behaviour plans and medical appointment support.
Your Role
- Case management and coordinating aspects of care for supported individuals
- Develop relationship with supported individuals with complex medical or clinical needs.
- Attend critical medical appointments
- Ensure that proper medical care is received
- Assist with training, onboarding, and providing delegation for medication administration to unregulated healthcare professionals
- Regular review of clinical documentation
- Oversight and review of data collection and behavior support plan adherence
- Attend case conference calls/meeting with the funder
- Participate in team meetings and ensures the team has a clear understanding of the expected client care as required
- Ensure high quality standards in care in the homes; continuity of care provision and quality and safe client services are in place
- Participates in all clinical aspects of onboarding
What you will need to be successful
- Registered with the New Brunswick Provincial College of Nurses, in good standing
- Proof of Liability Insurance
- Valid CPR/First Aid
- Experience in the Developmental and Mental Health sector an asset
- Medication Administration Training and Experience
- Valid Class 5 Drivers License
- Strong knowledge of the health care system, and experience in home and community care
- French Bilingual
