Job Summary

Our Rehabilitation Assistant report to the Client Services Coordinator and are expected to deliver a high standard of care that allows our clients and patients to remain safe and comfortable in their own home environment. The services provided will be detailed in the personalized care plan.

Responsibilities

  • Support clients with exercises, mobility training, and rehabilitation activities
  • Implement therapy plans developed by physiotherapists and occupational therapists
  • Monitor client progress and report observations to supervising therapists
  • Assist with activities of daily living where appropriate
  • Set up and maintain therapy equipment in the home
  • Document visits and communicate effectively with healthcare professionals and family members
  • Daily documentation and charting in electronic chart


Qualifications

  • Able to proficiently communicate in English (verbal and written)
  • Ability to speak other languages is considered an asset,
  • Ability to travel to and from clients by transit or by car
  • Completion of a recognized Physiotherapy/Occupational Therapy Assistant (PTA/OTA) diploma or equivalent
  • Experience in homecare or community health (preferred)
  • Ability to work independently and manage a flexible schedule
  • Strong interpersonal and communication skills
  • Access to a reliable vehicle and a valid Ontario driver’s license
  • Clear Vulnerable Sector Police Check
  • Access to internet/mobile phone
  • Negative TB skin test or chest x-ray
  • Current CPR and First Aid Certificate
