An opportunity exists for a casual Case Coordinator Acquired Brain Injury (ABI) to join the Brain Injury Program at the Vernon Community Care Health Centre in Vernon, B.C.
We are looking for a candidate who has strong case management skills, knowledge of comorbidity as it relates to mental health issues, a systems thinking approach to practice, and an interest in Acquired Brain Injury.
The ideal candidate is a skilled collaborator and communicator who thrives in a multidisciplinary environment, with experience supporting clients with complex neurological, traumatic, and rehabilitative conditions. The successful candidate will bring a strong understanding of community and acute care systems and are adept at navigating and coordinating services to support optimal client outcomes. With a client- and family-centered approach, the successful candidate is passionate about advocacy, education, and resource linkage, and is committed to facilitating seamless transitions through the continuum of care for individuals with Acquired Brain Injury.
What will you work on?
The Case Coordinator Acquired Brain Injury (Neuro Rehabilitation), in consultation with the client, family and interdisciplinary team members, provides case coordination to initiate and develop a plan of care, counsel client and family on issues that have impact on their care, refer to other community resources as appropriate, and to evaluate care plans. Provides and coordinates effective, individualized assessments and treatments for clients with complex neurological, traumatic and rehabilitative conditions into a practical continuum of care. Provides effective coordination and linkage between acute care and community services such as the Brain Injury Association, ICBC, private practitioners and rehabilitation programs, to facilitate effective discharge planning for clients with Acquired Brain Injury (ABI) and other complex neurological/rehabilitation conditions. Provides support as required to the ABI steering committee and/or ABI leadership group.
Scheduling Information
Hours of work are Monday to Friday from 08:30 to 16:30 hours. This is a Casual opportunity. Casual means there are no guaranteed hours and work requirements can vary from 0 - 37.5 hours per week.
