As a member of the Cool Aid Health Centres multidisciplinary team, the Health Outreach Clinician accepts referrals from the team relevant to the patient's social determinants of health. Utilizing the confidential electronic medical records system, the health outreach clinician extracts relevant medical information to direct and enact diagnostic and clinical treatment, and collaboratively work across disciplines to facilitate wrap-around services.

A narrow selection of the larger scope and responsibilities for this position are as follows:

  • Maintains patient records by charting patient information and assessments via designated electronic medical record and documenting patient interactions to ensure continuity of care.
  • Conduct chart reviews in the confidential electronic medical records system to guide subsequent case planning and treatment.
  • Provide relevant collateral information to the healthcare team when conducting collaborative patient assessments.
  • Promote and facilitate patient independence by modelling, counselling, and teaching life skills within a psychosocial rehabilitation model.
  • Organize work, set objectives, and establish priorities. Manage time and resources to implement care plans and promote cooperation among the interdisciplinary team and collaborate across disciplines.
  • Integrate best practice and current research evidence to support professional practices decisions and actions with strong competency in harm reduction, trauma informed practices and evidence-based approaches.
  • Ability to integrate and evaluate pertinent information from medical and non-medical sources to problem-solve and provide fulsome insight into patient needs in care plan creation and applications for various supports.
  • Complete applications for various Ministry benefits (i.e., Income assistance and Person with Disabilities benefits).
  • Complete applications for federal benefits such as Employment Insurance.
  • Initiate housing applications, assist patients with acquiring all the necessary documentation, e.g., Identification, bank statements, references etc.
  • Provide appropriate case management and follow up to ensure applications are completed in a timely manner.
  • Coordinate, mediate, and advocate on behalf of patients in order to obtain and/or maintain housing, health services, community services and other relevant supports.
  • Participate in staff and community meetings and in the day-to-day operations of the program, and field program inquiries as needed.

Qualifications

Education

  • Baccalaureate Degree in a health-related discipline from an approved post-secondary institution.

Experience

  • Minimum of two years of related work experience including working with individuals facing physical and mental health challenges, substance use, homelessness, poverty and/or other barriers.
  • A minimum of one year of service planning experience; or an equivalent combination of education, training, and experience.

Requirements

  • Completion of the Ministry of Public Safety and Solicitor General Criminal Record Check.
  • Class 5 driver’s license, and clean driving abstract.

Job Information

  • Wage rate: $33.26 - $35.95 per hour.
  • Work Location: 713 Johnson Street – Cool Aid Health Centre, outreach sites, and mobile clinics.
  • Work Days: Monday – Friday.
  • Shift Times: 9:00 am – 4:00 pm. Please note that some flexibility may be required.
  • Weekly Hours: 35 hours.
  • Union: Exempt.
  • Benefits: Eligible for 100% employer paid benefits.
  • Job Type: Full time - Temporary until September 2026.
