Role Description
This is a full-time role for a Professional Practice Clinician specializing in Wound, Ostomy, and Continence care. The Clinician will be responsible for assessing, planning, implementing, and evaluating patient care closely with the Most Responsible Physician. Day-to-day tasks include conducting patient assessments, developing individualized care plans, providing wound, ostomy, and continence education, and supporting the clinical team with ongoing patient care. The role also involves leading quality improvement initiatives, patient and staff education, policy and program development to enhance patient outcomes and experiences.
Qualifications
- Current registration with the College of Nurses of Ontario required
- Certification as a Nurse Specialized in Skin, Wound, Ostomy and Continence (NSWOCC) care required
- Completion of a Master Degree in Nursing preferred
- Current BLS required
- Membership in specialty related organizations e.g. Canadian Association of NSWOCC , RNAO
- Minimum 5 years in wound, ostomy and continence required
- Competency in sharp wound debridement
- Demonstrated knowledge of best practices, clinical practice guidelines and relevant clinical assessment tools and treatments
- Experience providing education to staff, patients and health professional regarding wound, ostomy and continence care
- Demonstrated ability to function effectively as part of an interprofessional team
- Ability to travel between the three hospitals as required

- Ability to work independently, with individuals and groups effectively

- Demonstrated ability to prioritize/organize multiple demands
- Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint and Visio required
