Person-and family-centered care (PFCC)
Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.
Location: Work available in Kingston, Ganonoque, and Belleville (must agree to at least 2 locations)
Currently looking for part-time (approx. 15 visits per week), and requires daytime weekday availability

Duties And Responsibilities
- Implement Care Plans: Execute social and wellness care plans under the supervision of a Licensed Social Worker. Provide in-person and virtual support for wellness checks, food and housing security, and social connections.
- Community and Resource Management: Follow up on community service referrals, manage patient transportation, and advise clients on available resources. Facilitate referrals and assist in discharge planning when social work intervention is no longer needed.
- Caseload and Record Management: Efficiently manage caseloads and maintain treatment records in compliance with the Ontario College of Certified Social Workers and Social Service Workers standards.
- Client Support and Education: Use a client-centered approach to address psychosocial issues, provide relevant educational resources, and participate in quality improvement initiatives.
- Professional Development and Collaboration: Engage in ongoing professional development, maintain confidentiality, and build positive relationships within the interdisciplinary healthcare team. Adapt to a dynamic environment and perform additional tasks as assigned.
- Social Service Work diploma
- Current Member (in good standing) of the Ontario College of Social Workers and Social Service Workers.
- Current CPR certificate (Basic Rescuer)
- Clear Criminal background (Police) clearance with Vulnerable sector screening
- Valid driver's license, auto insurance and reliable vehicle & must carry two million dollars in automobile insurance coverage if utilizing a personal vehicle for appointments.
- Possess general computer skills with access to a computer with appropriate password protection and internet capabilities.
- Minimum of two years of community-based therapy experience (preferred).
- Exceptional interpersonal skills;
- Ability to work independently and lead a team;
- Excellent report writing and communication skills;
- Able to remain calm under pressure and role model composure
- Solid knowledge of the principles, practices, and methods of community care and service delivery.
