HOURS OF WORK:

The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.

WHAT YOU’LL DO:

Acute and Chronic illnesses can have a significant life changing effect on individuals and their families. Social work provides service to patients and their families focusing primarily on the psychological and the emotional wellbeing of individuals as they adjust to the changes in their health. Social Work advocates on behalf of patients and families as they move through the health care system. Responsibilities include biopsychosocial assessment, problem formulation, assisting in the development, implementation and ongoing monitoring of patient treatment plans, crisis intervention, and knowledge of community resources. In addition, social work is an educational resource to patients, families, staff and community agencies. The Social Worker practices in accordance with the OCCSWSSW Professional Standards.

This Social Worker position will work collaboratively with the Pediatric Oncology team (POGO), and the entire inter-professional Women’s and Children’s team to provide comprehensive care and support for patients and families dealing with pediatric cancer. This position also involves helping families navigate the healthcare system, understand their treatment options and cope with the challenges of a cancer diagnosis. This includes ambulatory clinic support, outreach support, telephone consultation and support, and inpatient support when necessary.

WHAT YOU’LL BRING:

  • MSW, RSW or equivalent
  • Three years of Social Work Experience in Pediatrics in an acute care hospital setting
  • Member of the Ontario College of Certified Social Workers and Social Service Workers (OCCSWSSW) in good standing.

WHO YOU ARE:

  • Understanding of paediatric chronic disease management preferably in oncology
  • Paediatric social work experience in an Outpatient Setting
  • Strong understanding of the psychosocial needs of pediatric oncology patients and their families
  • Knowledge of community resources and services available to pediatric oncology patients and their families.
  • Ability to provide culturally competent care to diverse patient populations.
  • Strong advocacy skills and a commitment to patient-centered care.
  • Crisis and trauma counselling with children and/or their families.
  • Working in a therapeutic environment with children and families and comfortable with a family-centred and child-centred approach.
  • Family assessments and counselling.
  • Family-Based Motivational Interviewing
  • Understanding of relevant child welfare legislation, i.e. Child and Family Services Act and Child Abuse Review Teams.
  • Willingness to work flexible hours in order to accommodate patient care conferences, clinical rounds, staff meetings, community meetings that may occur outside the normal working hours.
  • Ability to work independently and collaborative with a large inter-professional team and community teams in a hospital environment
