Job Summary
The Pharmacist shall be responsible for the accurate clinical assessment and dispensing of pharmaceutical preparations according to medication order; consulting with physicians, nurses and other health professionals and furnishing information concerning appropriate drug therapy; maintaining all records and files as required by law and pharmacy policy, and the daily security of the pharmacy department. Pharmacists may be required to train to care for diverse patient populations, including but not limited to oncology and paediatrics. The pharmacist complies with the Occupational Health and Safety Standards for the hospital and is proactive in promoting a safe work environment.

Qualifications
- Experience in a hospital pharmacy environment – preferred
- Previous anticoagulation experience – preferred
- Preference will be given to individuals who have completed a certificate program in anticoagulation
- Excellent written and oral communication skills;
- Excellent interpersonal skills and initiative;
- Excellent organizational skills;
- Able to work effectively as a member of an interdisciplinary health care team;
- Commitment to standards of excellence and the concept of accountability;
- Commitment to and participation in Quality Enhancement activities;
- General good health and a level of fitness appropriate to the demands of the position
- French language skills an asset
- B.Sc. Phm. Degree – required
- Pharm D - preferred;
- Residency - preferred;
- Licensed or eligible for licensure to practice pharmacy in the province of Ontario
- In good standing with the Ontario College of Pharmacists with Part A (direct patient care) license with OCP
