Position Information

This position includes clinical nursing support with the TOH Stroke Prevention Clinic and includes coordination of regional network stroke prevention activities in Champlain.

The Champlain Regional Stroke Network (CRSN) is one of 11 stroke networks in Ontario. Regional stroke networks are a part of the Ontario Stroke System and are accountable for the leadership, development, implementation and coordination of stroke care within their region and the provision of stroke care based on best practices and evidence. The CRSN is a key healthcare partner supporting coordination of stroke care throughout the Champlain LHIN and across all points in the stroke care continuum.

The role of the SPC is to ensure that individuals who are at high risk for stroke receive evidence-based care founded on best practices. Stroke Prevention Clinics provide an integrated, comprehensive, interdisciplinary approach to stroke prevention. Their main objectives are to reduce delays and inefficiencies in risk factor management of high-risk patients and to facilitate timely access to interventions.

Rotation/Shifts

Days, 8 hours shifts. For information purposes only - The Hospital reserves the right to change.

Basic Requirements

  • Certificate of registration from the College of Nurses of Ontario;
  • Masters of Science in Nursing or Education, or in a related discipline with an undergraduate degree in Nursing;
  • Member of the Registered Nurses Association of Ontario;
  • Minimum of 5 years nursing experience;
  • Minimum of 3 years in neuroscience and/or stroke care;
  • Experience with quality improvement and knowledge translation skills;
  • Sound judgement and problem-solving abilities;
  • Positive interpersonal and stakeholder engagement skills;
  • Excellent clinical knowledge in stroke care;
  • Excellent teaching and consulting skills;
  • Excellent public speaking and written communication skills;
  • Clinical expertise related to patient care and administrative, educational and research matters;
  • Knowledge of computers including MS Office;
  • Excellent organizational, interpersonal, problem solving and decision making skills;
  • Demonstrated leadership ability;
  • Expert communicator;
  • Ability to learn and apply hospital policy and procedures, relative legislation, collective agreements, Canadian Hospital Accreditation Standards, Professional Disciplines Standards for Ontario;
  • Eligible to work in Can ada;
  • Ability to travel to regional hospitals within Champlain.


Preferred Qualifications

  • Experience presenting at national & International Conferences;
  • C.N.A. Certificate in Neuroscience;
  • Proficiency in English and French – oral expression (advanced level) and comprehension (advanced level).
