Job Description

Reporting to the Supervisor SNAP (Stop Now and Plan), the Social Worker SNAP is responsible to provide evidenced-based treatment delivery and case management services, including groups, individual and family therapy to address behavioural and emotional difficulties for children ages 6 – 12 and their families. As the primary clinician on a multidisciplinary team of Child and Youth Workers, the Social Worker also provides a clinical lens to treatment, assesses client risk factors and develops safety plans with caregivers. The position currently requires a combination on in-person and virtual service.

The role of Social Worker SNAP has been positionally assessed in the 'Roamer Worker’ classification, defined as 3 days in person per week and as eligible for a Compressed Work Week.*


Duties & Responsibilities:

  • To facilitate groups for parents to teach parenting strategies and emotional regulation skills. To facilitate groups for children to teach emotional regulation skills.
  • Provide individual and family support related to behavioural and emotional difficulties. Also, when appropriate, provide individual and family therapy for clients/families when their needs exceed support around implementing SNAP strategies due to factors such as complex mental health, family conflict, and divorce/separation.
  • Perform risk assessment and develop safety plans if required for every family in the SNAP program.
  • Provide service to families waiting for service. Based on the risk assessment follow-up regularly (every 30 – 90 days) with the family until service begins.
  • Case management for assigned families. This includes welcoming them to the SNAP program, developing goals and treatment plans, maintaining contact throughout their service until discharge, consulting with other treatment providers (internal and external) about the services for the family, ensuring all required documentation and program measures are completed.


Qualifications:

  • A Master’s Degree Social Work is preferred.**
  • Candidates with a Bachelor’s Degree in Social Work, with extensive and relevant experience may also be considered.
  • Must be registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • A minimum of 3 years relevant experience working in a mental health setting, is preferred.
  • Excellent clinical assessment, formulation and treatment planning skills is essential.
  • Training or experience in Cognitive Behaviour Therapy or Dialectical Behaviour Therapy will be considered assets.
  • Experience with providing a broad range of clinical services and demonstrated effective work with a child and adolescent population who present a variety of clinically challenging issues.
  • Computer competency and experience working with diverse cultures are considered as assets for this position.
  • Travel is required for this position; the successful candidate must have a valid Ontario driver’s license, access to a reliable vehicle and be willing to obtain automobile insurance which includes driving for business purposes with a minimum liability of one million dollars.
