We utilize a person-and family-centered care (PFCC) approach, which recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.
Position Summary
We are currently seeking part-time Rehabilitation Therapists responsible for providing supervision and support to individuals with acquired brain injuries (ABIs). This position is based out of our Supported Living Apartment (SLA) in Toronto, ON where each client resides in a fully equipped private suite. We provide 24-hour support and programming for clients with ABIs and concurrent physical, mental, behavioural, and/or substance use challenges. Goals you’ll be supporting may include increasing levels of independence and participation in life activities, community reintegration, social reintegration, and avocational/volunteer activities to name a few! Overall, as a Rehabilitation Therapist, you will be part of a team that endeavors to deliver specialized care in order for clients to make strides in their recovery.

Qualifications
- Post-secondary studies in social sciences or healthcare related areas
- G driver’s license with a clean driver’s abstract, access to a vehicle and the proper insurance to transport clients in own vehicle is required (you will be transporting clients within the community)
- Availability to work on weekends, days, evenings, occasional overnights and some holidays
- Please note: this is a part-time position and hours fluctuate according to client census
- The ideal candidate will also possess the following:
- Experience working with individuals with mental health issues, dual diagnoses, behavioural challenges, chronic pain, or addictions
- Strong conflict resolution and problem-solving skills, including managing challenging behaviours (e.g., aggression, elopement, inappropriate conduct)
- Ability to work independently, take direction, and collaborate effectively with a team
- Willingness and ability to perform personal care, medication assistance, ADLs, administrative tasks, light cleaning, advocacy, and therapy implementation
- Excellent communication and interpersonal skills
- Physically capable of bending, stretching, and lifting
- Highly organized with the ability to complete tasks within set timeframes
- Proficient with computers and secure email systems
- Committed to ongoing learning and professional development
- Work with individuals with acquired brain injuries while assisting them with increasing levels of independence through individualized rehabilitation programs (responsible for up to 5 clients at a particular time)
- Complete documentation following company and provincial guidelines
- Follow the designed rehabilitation program as accurately as possible
- Assist clients with activities of daily living (ADLs); performing personal care, medication management, and various forms of transfer tasks
- Accompany clients on community outings as needed via external transport services or your own vehicle
- Utilize appropriate Crisis and Behavioural Management strategies
- Collect required clinical data through documentation, tracking, etc.
- Complete research to assist with enhancing client programs
- Other clinical and administrative duties as assigned
- Provide flexible availability (evenings, weekends, overnights, occasional holidays)
