The Job Summary:
As a Long-Term Care (LTC) Social Worker, you will assess and determine resident care needs and eligibility, provide access and referrals to community services, and engage with residents, caregivers, and healthcare practitioners prior to bed offer. This role supports individuals on the waitlist by offering comprehensive assessments, care planning, psychosocial support and resource navigation, advocacy and coaching and assistance with completion of appropriate government service’s needs. By addressing the unique needs of waitlisted candidates and their families, the LTC Social Worker ensures a seamless admission and discharge transitions for LTC, fosters collaboration, promotes informed decision-making, and enhances the overall experience with a focus on compassion, accountability, respect, and excellence.
Key Accountabilities & Success Criteria:
- Conduct Long Term Care (LTC) resident waitlist assessments ensuring compliance with Fixing Long Term Care Act 2021 (FLTCA) and a smooth transition into LTC in collaboration with the LTC team.
- Support advanced care planning, prioritizing residents’ preferences and end-of-life care needs.
- Facilitate access to financial resources and guide families through funding options and tax-related processes.
- Educate and assist with Power of Attorney, capacity assessments, and Substitute Decision-Making arrangements.
- Coordinate collaborative care plans with families, healthcare providers, and community partners.
- Maintain clear communication and effective relationships with community agencies, health care facilities, residents and families to ensure transparency and trust.
- Collect and utilize feedback to enhance assessment processes and care experiences.
- Provide accessible information and resources to help residents and families make informed decisions.
- Provide information to potential and current residents and/or families regarding the admission/discharge process.
- Act as a liaison with community agencies and partners regarding the placement and support of prospective residents.
- Facilitate and review for all waitlisted residents in collaboration with the LTC Admissions Coordinator, LTC Resident Support Associate and OHaH Care Coordinators and LTC Management.
- Lead and coordinate aspects of the resident admission including ensuring staff are available for communicating with new admission/SDM and communicating with clinical staff in preparation for the admission or discharge to alternate location.
- Collaborate with the multi-disciplinary team to coordinate the resident’s admission and or discharge.
- Coordinate follow up with resident and Substitute Decision Maker (SDM) following admission and or discharge for satisfaction and feedback.
- Notify Director of Long-Term Care, Assistant Director of Long-Term Care, Registered Staff and interdisciplinary team of all pertinent information to establish a resident care plan
- Promote an environment that encourages and supports change using change leadership theory.
- Contribute to the organizations efforts and initiatives to enhance SJHCG’s missions, vision and values by developing a culture of resident and staff safety through accountability, reliability, respect, trust and teamwork.
- Perform duties in accordance with all applicable legislative requirements (i.e. Ministry of Long-Term Care Act/Reg., Ministry of Labour, Public Health Act, etc.).
- Work in compliance with the Occupational Health and Safety Act and its regulations, reporting hazards, deficiencies, and contraventions of the Act in a timely manner, taking action to address any items.
- Provide comprehensive psychosocial assessments on all referred residents from the LTC Resident Support Associate, Admissions Coordinator from the LTC Admission waitlist.
- Formulate treatment plans based on the psychosocial assessment and select an appropriate method of intervention within the framework of social work knowledge and skills in collaboration with OHaH.
- Assist residents in liaising with community and appropriate government services e.g. Pension office, ODSP, Regional Social Services. Also assist in obtaining funding for necessary items e.g. wheelchairs, clothing, shoes, etc. in collaboration with OHaH and other external partners
- Conduct cognitive/mood testing as required in collaboration with OHaH.
- Act as an advocate on behalf of residents and their families.
- Provide consultation to physicians, and Interdisciplinary Team members regarding the social and emotional factors which bear upon the resident’s diagnosis, treatment and care plan upon request from OHaH when LTC bed offer occurs.
- Maintain current and proper records and documentation necessary for effective resident treatment and collaboration.
- Complies with resident safety programs, and processes including identifying and reporting adverse events/near misses.
- Attends and participates in appropriate safety training or education updates (ie. hand hygiene, emergency code reviews).
- Identifies unsafe conditions or practices and takes appropriate action to address them including reporting or removing hazards.
Qualifications:
- Bachelor Degree in Social Work from a recognized School of Social Work.
- Master’s Degree in Social Work (preferred).
- Membership in Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- 3-5 years experience working in a health care setting.
Skills & Abilities:
Demonstrated conflict resolution and crisis intervention skills with clients and their families.
- Strong knowledge of LTC related legislation preferred.
- Detailed knowledge of community services/resources.
- Excellent verbal and written communication skills.
- Proficient in Microsoft Office with the ability to navigate computer systems confidently for accessing Personal Health Information, documenting notes and performing related tasks.
- Ability to take initiative and work independently and as a part of a team.
- Excellent organizational skills.
- High level of maturity, professionalism and acceptance of personal responsibility.
- Excellent attendance record and ability to maintain same.
