Job Title: Nurse Practitioner
Location: Niagara Falls Community Health Centre
Work Schedule: Full-time, 37.5 hours/week (flex time and part-time considered)
Salary: $60.10 - $66.10/hr + HOOP pension, and other benefits listed below

Key Responsibilities:
- Provide holistic, client-centered care to specialized populations with barriers to care, including pediatrics, refugees, newcomers to Canada, trans care, & palliative care.
- Conduct assessments, diagnoses, planning, implementation, and evaluations in-person and virtually to your own roster of patients.
- Perform home visits for clients unable to visit the clinic.
- Collaborate with interdisciplinary teams to deliver high-quality care.
- Engage in quality improvement, research initiatives, and student mentorship.
- Utilize Electronic Medical Records (EMR) efficiently for documentation.
What We’re Looking For:
- Degree in Nursing from a recognized institution; current extended class registration with the College of Nurses of Ontario.
- Minimum of 3 years of clinical experience, preferably in a community setting.
- Passion for working with socially and medically complex patients.
- Excellent communication and interpersonal skills.
- Proficiency in EMR and virtual care tools.
- Commitment to continuous learning and professional growth.
- Ability to work flexible hours and participate in on-call schedules.
- Valid Ontario Driver’s License and access to a reliable vehicle.

- Police Reference Check.
