The Manager of Clinical Practice is responsible for the daily operations of the Bundled Care Program. Completes intake assessment and develops care plan for newly admitted patients. Provides effective leadership and clinical support to care team. Responsibilities include planning and executing activities related to service delivery and performance management of Nurses and Personal Support Workers. In conjunction with the Clinical Manager, Bundled Care Programs, monitors contractual performance through analysis, tracking, trending and implements strategies for performance improvement.
Primary Responsibilities
You will work to meet and mentor the diverse needs of the clients and caregivers you serve in your community by providing a wide range of services and support.
- Supports the Bundled Care Program by ensuring compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
- Collaborates with all members of the Clinical Care Team including, Nursing, Personal Support Workers, Allied Health and Social Workers to provide a holistic service approach to community patient care.
- Collaborates with external partners to facilitate safe patient care transitions and planning with Home Community Care Support Services (HCCSS), Hospital partners and other community support services as required.
- Effectively manage, record and follow up on patient and family concerns.
- Attend meetings as necessary for case conferences as required.
- Performs case reviews and monitors and analyzes the achievement of client care goals and outcomes of care by efficient case management.
- Ensures that services are delivered according to best practices and established pathways and protocols.
- Monitors and analyzes the intake process, priority codes and refusals and implements processes to improve rates.
- Ensures all aspects of the client assessment process are completed.
- Works in conjunction with the Clinical Manager to manage the reporting metrics and outcomes of program and patient care to efficiently report, track and communicate with data management team.
- Escalates any care concerns or service level concerns to Clinical Manager for further review.
- Complete patient home visits in instances of training, performance evaluation and to support skills training in the home.
- Builds and designs care plans in collaboration with hospital partners to facilitate safe patient transitions from hospital to home.
- Provide mentorship, supervision, performance management to personal support and /nurses supporting your program.
- Deliver the highest quality home health care with compassion, excellence, and reliability
- Facilitates and maintains effective communications strategies to ensure quality client care and foster positive relationships, both internal and external to the agency.
- Participate in ongoing internal and/or external continuing education activities
- Must be available to work flexible hours which may include days, evenings and weekends.
- Participate in on-call schedule rotation as required.

Job Qualifications
- 5+ years’ experience in health care, preferably home health care & hospital.
- Current license in good standing as a Registered Practical Nurse, in the province of Ontario.
- Graduate of an accredited and approved program.
- Completed Continuing Education Requirements (CEUs) as required by regulation
- Solid knowledge of the principles, practices, and methods of community nursing and service delivery.
- Knowledge of Ontario community health system navigation and patient discharge care planning recommended.
- Exceptional business acumen and ability to collaborate with hospital partners, community partners and stakeholders at all levels.
- Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format
- Exemplifies characteristics of Bayshore Health Care: compassion, excellence and reliability.
- RAI Tool experience and knowledge an asset.
- Evidence of current CPR certification
- Have valid driver’s license and reliable vehicle for commuting
- Knowledge of health & safety and infection prevention and control.
- Demonstrated leadership skills
