The Algoma Geriatric Clinic (AGC) provides specialized geriatric services including but not limited to inter-professional comprehensive geriatric assessments to the elderly population of Algoma and facilitates a plan of care to enable the elderly person to age at home with the appropriate supports.

The Sault Area Hospital is currently recruiting a Full-Time Occupational Therapist for the new Community Geriatric Rehabilitation Program.

DUTIES:

  • Provides comprehensive geriatric assessment, program planning, treatment, follow-up and program evaluation to clients in the community.
  • Provides appropriate education, consultation, recommendation and treatment plans in consultation with the client, family/caregivers, physicians and other health care professionals to promote optimal level of occupational performance.
  • Determines the need for in-home assessment or linkage to community resources in consultation with the team.
  • Provides effective written and verbal communication according to hospital and departmental guidelines and college standards.
  • Works in a manner to ensure safety to oneself, clients, colleagues and the work environment.
  • Provides educational programs for staff and groups in the community.
  • Maintains up to date knowledge of best practices in geriatrics and psycho-geriatrics, especially as it relates to occupational therapy.

QUALIFICATIONS:

  • Graduate of an accredited university program in occupational therapy that meets academic and clinical requirements for registration with COTO.
  • Current registration with the College of Occupational Therapists of Ontario (COTO).

EXPERIENCE:

  • Minimum 3 years’ clinical experience
  • 2 years of experience working with the older adult population.

QUALIFICATIONS:

  • In-depth knowledge of techniques for assessing and treating basic co abilities and activities of daily living.
  • Working knowledge of the psychological, physical and functional implications of the aging process.
  • Demonstrated ability to assess cognitive functioning and mental status in the elderly.
  • Demonstrated adaptability and flexibility to respond to emerging and dynamic program needs.
  • Excellent organizational, written and verbal communication skills.
  • Applicable ability and experience working with Microsoft programs and Meditech.
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
  • Extensive knowledge of community resources preferred.
  • Proven ability to liaise with referral sources, community agencies and other team members to enhance positive patient outcomes.
  • Demonstrated commitment to excellence in patient/family care.
  • Proven ability to maintain and promote positive inter-professional team relationships.
  • Demonstrated behaviour consistent with the hospital’s Standards of Performance and iCcare values.
