Position Summary

Clinical Pharmacists provide direct patient care to manage the patient’s medication therapy needs, ensuring that drug therapy is indicated, safe, and effective. The Clinical Pharmacist will work closely with patients, their family members and other health care providers to help patients achieve positive health outcomes. The incumbent will actively engage in quality improvement and patient safety initiatives in various patient care areas and within the pharmacy department. Additionally, Clinical Pharmacists participate in the development, revision, and implementation of standards and protocols pertaining to drug therapy and pharmacy practice. Pharmacists work closely with a committed team of healthcare professionals. Working in a CPOE environment, our pharmacists are actively involved in reviewing medication orders, maintaining patient profiles, clinical pharmacotherapy monitoring and providing clinical support to medical staff.

Position Requirements

  • Licensure in part A, Ontario College of Pharmacists
  • Direct patient care experience in an inter-professional health care setting (e.g. acute care hospital or Family Health Team), preferred – minimum of one (1) years experience in a hospital inpatient pharmacy

Position Responsibilities

  • Successfully able to perform their function in accordance with prescribed standards that focuses on the priority of People Safety and the delivery of the highest standard of Quality Care.
  • Demonstrate the physical and mental capabilities necessary for proper and efficient performance of the essential duties of the position.
  • Focused practice based on meeting OCP hospital pharmacy accreditation standards
  • Strong communication, leadership, problem solving and organizational skills
  • Demonstrated reliable attendance and punctuality
  • Excellent interpersonal skills and ability to work collaboratively in a team environment

Preferred Requirements

  • Completion of an accredited hospital residency program, preferred
