The shift pattern for this post is Mon-Fri 0900-1700.

This is a one year fixed term post based within the Queen Elizabeth University Hospital Glasgow.

This role has arisen from a collaborative working agreement between NHS Greater Glasgow and Clyde and Pfizer to improve the care of patients with cardiac amyloidosis. Amyloidosis is a rare, multisystem disease associated with high symptom burden, poor quality of life and reduced life expectancy. The presentation is variable, it eventually progresses to end stage heart failure and delays in diagnosis are common. There are inequities in care for patients. Importantly, there are increasing novel treatment options for patients with amyloidosis. Patients with suspected cardiac amyloidosis are to be diagnosed promptly and managed through multidisciplinary teams delivering high quality specialised care as close to home as possible.

The aim is to establish a coordinated clinical pathway through multi-disciplinary teams, ensuring timely access to diagnostics and treatments, while addressing unmet needs in evaluation, research, and quality improvement. The over-arching aims are to support the delivery of a comprehensive plan to educate, improve disease awareness, develop operational capabilities and understand and deliver patient focused services with equity, all targeted at improving clinical pathways across Greater Glasgow & Clyde The newly appointed Multi-Disciplinary Teams (MDT) Co-Ordinator/ Administrator will also work closely with a newly appointed Advanced Nurse Practitioner to support development of the amyloidosis service. The MDT Co-ordinator/ Administrator supports and improves the development of the service by identifying areas that can improve the experience and outcome of care for amyloidosis patients. The MDT Coordinator/ Administrator will work with the Multi-Disciplinary Teams to ensure the service is well structured, runs effectively and that all patients are treated and discussed according to evidence based protocols.

Key Tasks Include

  • Maintain databases to track the progress of patients with suspected and confirmed amyloidosis through multidisciplinary specialist services
  • Co-ordinate MDT meetings to facilitate communication and the provision of planned care.
  • Ensure there is clear definition of roles and responsibilities for MDT members.
  • Liaise with departments involved in providing care and services, to enable planned patient treatment to progress smoothly and in a timely manner.
  • Accepting MDT referrals and liaising with the responsible clinician on suitability for MDT discussion.
  • Collect data relating to urgent GP referrals, referrals from other hospitals in the West of Scotland, and pre existing patients with amyloidosis and prepare statistical reports
  • Ensure that GPs and other specialists are notified of discussion and decision of MDT within relevant time scale.
  • Ensure that systems are in place to deliver a quality supporting and administrative service to the teams.
  • Assist within the wider cardiology team where required.
