The Social Worker for Integrated Care Solutions will be able to adapt social work principles and practices to the community setting. This includes, but is not limited to, assisting the community adult client to maximize social, emotional, and cognitive functioning that promote optimum independence and improve quality of life. The Social Worker will conduct comprehensive psychosocial assessments, develop and implement individualized care plans, and provide ongoing support and advocacy for clients. They will collaborate with interdisciplinary teams to ensure holistic care and connect clients with community resources and services. The role involves crisis intervention, case management, and education, aiming to empower clients and their families to achieve their goals and enhance their well-being.

Community Based Position - Per visit rate and travel units, requires Monday to Friday daytime availability

Duties And Responsibilities
- Communicated with clients, families/caregivers, service providers and other professionals to collaborate and coordinate services.
- Maintain and manage a caseload in an efficient and effective manner ensuring priority code timeframes are met.
- Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect them.
- Utilize a client-centered approach to identify, prioritize and monitor potential psychosocial problems and goals.
- Establish, implement, evaluate and adjust the psychosocial treatment plan in collaboration with the client and/or family caregivers, care
- coordinator and other community partners
- Enhance or teach coping skills e.g. relaxation, stress management, anger management, healthy grieving.
- Advise clients/caregivers on the availability of community resources and contact agencies on their behalf when appropriate e.g. Federal, Provincial,
- Municipal and Local agencies in relation to income security, pensions, financial assistance, etc.
- Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
- Recommend and facilitate referrals to support services within the community.
- Comply with continuous quality improvement initiatives of Bayshore Home Health
- Developing a flexible, responsive system to meet changing and competing demands
- Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body.
- Liaise with other Case Managers, funders or Government & hospital programs as required.
- Positive work and attendance performance essential.
- Participate in ongoing internal and/or external continuing education activities.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
- Liaise with Provincial regulating body as required.
- Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety
- Actively participates in and demonstrated support for continuous quality improvement initiatives.
- Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.

- Possesses a Master’s and/or Bachelors Degree in Social Work
- Prior experience within home assessments and treatments an asset
- Be an active member in good standing of the Ontario College of Social Workers and Social Service Workers
- Provide a police clearance that meets the Bayshore Home Health policy
- Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities
- Preferably a minimum of two years of community-based therapy experience.
- Preferably the ability to drive, hold a current, valid driver’s license and have access to a vehicle in good operating condition
- Must carry two million dollars in automobile insurance coverage if utilizing personal vehicle for appointments
- Hold a current CPR certificate.
