Bayshore Home Care Solutions, Brantford Branch, seeks a motivated Nurse Team Lead.

Full Time, permanent
Monday – Friday, plus on call rotation

Community nurse visits – 2 days per week
In-Office clinical support – 3 days per week

The Work You Will Do
As a Nurse Team Lead you will be very hands-on in the community, providing direct client care, supporting a happy, healthy well-balanced team of nurses and building your client base all while making a difference in the communities we serve.
Assessment Delivery
Visit your Clients and Nurses on a regular basis. This role is an active community position that requires regular home visits within the community. This role involves a high level of hands-on care, training and coaching including weekly onsite visits with clients and nurses in the community.
Leadership and Office Clinical Support
Support Clinical Manager with orientation and onboarding, support visits, and the development of skills and ongoing competencies
Support the Clinical Manager with investigations during client satisfaction/concern process
Support Clinical Manager with completion of home support initial visits to ensure care plans are individualized in a timely fashion
Visit clients and staff, providing hands on care as well as training, supervision, mentorship and support
Review and support the APR process of the nursing team to ensure targets are met
Audit and regulatory compliance for provincial and in-house best practices Combine an entrepreneurial spirit and hands-on leadership in the community through organized outreach
Human Resources
Assist human resources concerning recruitment and selection, orientation and training, discipline, and the probationary/annual performance review of reporting neighborhood health team members
Supervise and support nurses to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
Responsible for the safety of your team and we ask that you notify your immediate Supervisor of any Health & Safety risks or concerns
Training & Development
Coordinate and provide training and mentorship for nurses
Engage in ongoing education, continuous learning for yourself
Community Care
Provide hands-on Clinical/Nursing care
Build Client relationships
Support/Train and Mentor nurses in the Field while working with Clients
This position may have an occasional an after-hours support component
Experience with community outreach and relationship development, networking
Graduate of an accredited Provincial Nursing Program with current registration in good standing and valid CPR certification
Drivers license and reliable vehicle
Minimum of three years community nursing experience or as determined appropriate for branch needs based on skills/qualifications
Solid knowledge of the principles, practices, and methods of community nursing and service delivery to clients as well as program development, implementation and evaluation
Recent experience with progressive responsibilities, preferably in a community setting an asset
Experience in business development is an asset
Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and effectively lead a team; above average report writing and communication skills with both Field and Administrative employees.
