As a Social Worker in the Tuberculosis Program within the Communicable Diseases Division, you’ll play a vital role in delivering and coordinating social work services for individuals, families, groups and communities. You’ll support clients diagnosed with Tuberculosis facing other health challenges such as HIV/AIDS and chronic illness, and social challenges that may impact their treatment success, like housing and food insecurity, mental health, addictions, settlement issues and access to resources.
This role offers variety and purpose – no two days are alike. From conducting home visits and community outreach to collaborating with health professionals and advocating for clients, you’ll be at the forefront of building healthier, stronger communities in Peel.

What You’ll Do
- Provides social work services to individuals, families, groups and the community in line with Peel Public Health’s specific policies and procedures professional standards.
- Carry out work in diverse community settings – client homes, shelters, schools, community agencies and Peel Public Health offices.
- Conduct assessments and provide counselling, education, and guidance on socio-economic, settlement, financial, employment, and health-related issues.
- Perform independent home visits and assist clients/families in achieving goals related to health and communicable disease management (e.g., TB treatment completion).
- Deliver ongoing therapeutic counselling for clients facing complex challenges including addictions, sexual abuse, family violence, food and housing insecurity, and bereavement.
- Provide crisis intervention and make referrals to appropriate authorities (e.g., Children’s Aid Society) in accordance with legislation.
- Maintain accurate documentation and client records in compliance with divisional policies and professional standards.
- Collaborate with community partners such as schools, agencies and hospitals to support coordinated client care.
- Facilitates access to Region of Peel programs and services and advocate on behalf of clients and families
- Participate in case conferences, contribute to program development and evaluation, and support internal policy and resource creation and implementation
- Mentors new staff and students, deliver educational workshops within the community and present at professional conferences
- Appears and gives evidence in court and inquests when subpoenaed in consultation with management and legal services
- Supports Public Health and Regional urgent responses as required
- Demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.
- A Bachelor or Master of Social Work degree from a recognized university and membership in good standing with the Ontario College of Social Workers and Social Service Workers
- Experience working with challenging and high-risk client situations (e.g. addictions, sex work, victims of violence, street-involved individuals, mental health issues, housing insecurity, refugee and asylum claimants)
- Experience working with clients with communicable diseases including Tuberculosis and STI/HIV/AIDS
- Experience providing counselling related to social, health, settlement and housing issues using culturally appropriate approaches and trauma-informed practice.
- Understanding of Peel’s diverse communities and the unique issues affecting marginalized populations (refugees, recent immigrants, low-income, 2SLGBTQI+, racialized groups)
- Demonstrated experience in health promotion, community development and experience working in an outreach capacity
- Must possess a valid driver’s license and have access to a vehicle for work purposes.
- Excellent assessment and counselling skills
- Knowledge of and ability to access community and social service agencies
- Knowledge of relevant Regional and Provincial legislation including Substitute Decisions Act, MFIPPA, PHIPA and policies and procedures
- Strong interpersonal skills and the ability to handle challenging conversations and situations
- Creative problem-solving skills with the ability to develop innovative strategies that meet client needs.
- Flexible and organized; able to manage competing demands and respond to client needs as they arise.
- Ability to work independently and as part of a multi-disciplinary team
- Demonstrate a commitment to diversity, equity and inclusion through continuous development, modeling inclusive behaviours, proactively managing bias.
