About The Role

We are seeking a dedicated and compassionate Visiting Registered Nurse interested in a Full Time Permanent opportunity to provide high-quality nursing care to clients in their homes within the Greater Hamilton and Burlington area. This role is ideal for an experienced nurse who values autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families in a community setting while leveraging their diverse skillset.

Key Responsibilities

  • Conduct in-home assessments and deliver nursing care based on individualized care plans
  • Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions
  • Monitor patient progress and update care plans accordingly
  • Educate patients and caregivers on disease management, medication adherence, and health promotion.
  • Collaborate with interdisciplinary teams and task delegation to ensure continuity of care
  • Maintain accurate and timely documentation using electronic health records
  • Participate in on-call rotations as required (typically twice per month overnights)

Qualifications

  • Registered Nurse (RN) with current CNO licensure in good standing
  • Minimum of 2 years of nursing experience, preferably in ER or as a Visiting Nurse
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Valid driver’s license and access to a reliable vehicle with willingness to travel throughout the community (average of 75 visits Bi-Weekly)
  • Proficiency with electronic charting systems
  • LEAP Training, Palliative Certification, considered an asset
