About the role: The Hospital strives to deliver care that is centred on patients and families. Reporting to the Reporting to the Manager ‑ Ambulatory Mental Health and Supportive Care Services, the successful candidate will provide psychosocial assessments, counselling and crisis interventions for patients and their families and advocacy for patients needing immediate assistance while in hospital including necessary post discharge arrangements on in patient units. As part of a multidisciplinary health care team, the successful candidate will practice within the standards of the Ontario College of Certified Social Workers and the Mission Statement and Code of Conduct of the Thunder Bay Regional Health Sciences Centre
Applicants must have the following qualifications: * Honors Bachelor of Social Work degree * Current registration with the Ontario College of Certified Social Workers * Minimum of 2 years experience in an acute care environment
In addition, applicants must have skill and experience in the following elements of practice: * Relevant clinical experience with Chronic Disease and multiple/complex medical issues * Strong assessment and counseling skills * Excellent written and verbal communication skills * Displayed abilities in Critical thinking, problem solving and conflict resolution * Proven abilities to work well under stress and in a constantly changing environment * Experience in teaching acutely ill patients and their families * Displayed awareness of appropriate community resources * Experience practicing as an active participant on a multidisciplinary team * Experience working with people at end of life and in the setting of bereavement * Excellent knowledge of community resources is essential
Salary Range: $39.160 to 50.770
