Role Overview
The Case Manager, Registered Nurse provides case management support and care for the over all well-being of clients.
About The Team
This Case Manager, Registered Nurse (CM, RN) is a member of Cota’s Assertive Housing First Team (AHFT). AHFT is an interdisciplinary team of health care professionals comprised of a program psychiatrist, nurses, peer support specialists, addictions specialists, and mental health clinicians which provides support to adults (18+ years) who are homeless in the City of Toronto and are living with a serious mental illness. The interdisciplinary team works with individuals to find safe and affordable housing based on personal preferences and provides intensive interdisciplinary team supports to assist them in maintaining housing stability and their pursuit of personal recovery.

Upon successful completion of a three-month period of employment, permanent full-time and permanent part-time staff working more than 20 hours per week as well as temporary full-time (with a contract for at least 1 year) and temporary part-time staff (with a contract for at least 1 year and working more than 20 hours per week) will be eligible for employer-paid group insurance benefits. Primary Responsibilities
Clinical Skills
- Review referral information of applicants to the program, assess eligibility for the program and confirm acceptance of new service users into the program.
- Develop collaborative, supportive and therapeutic relationships.
- Complete comprehensive psychiatric and physical health assessment of the nature of each service user’s unique goals and needs through collaborative engagement with each service user, and their respective support networks.
- Provide guidance and assistance to each service user in relation to the development of both a personalized Safety Plan and a Recovery/Goal Plan.
- Provide health education to service users and team to promote health and well-being.
- Provide direct practical support, coaching and counsel to service users in their pursuit of identified goals and needs.
- Support administration of medication program in collaboration with AHFT psychiatrist, including administration and delivery of medications and administration of intramuscular injections.
- Assisting service users with emergency and/or crisis needs as required/appropriate.
- Documents all service user contacts/interactions in the appropriate chart according to Cota’s documentation guidelines and College standards.
- Assists with other data collection activities within the program, as determined by the Program Manager.
- Participates in team meetings, provides input and ideas for improving services, efficiencies and ongoing program planning.
- Supports creation of team schedule and service user appointments.
- Participates in any research-related activities.
- Oversee distribution of daily meds to team members
- Reconciliation of weekly meds
- Liaison with pharmacy
- In Office nurse with some local community visits and or visits throughout the GTA when necessary
Qualifications:
- Bachelor’s degree in nursing
- Registered with the College of Nurses of Ontario
- Minimum 2 years direct experience in community mental health nursing Experience working with clients with a concurrent disorder and those living in unstable housing situations or homelessness
- Experience working in a community agency an asset
- Conduct home visits and to exercise judgement with a high degree of autonomy.
- Establish supportive, trusting relationships with persons with a serious mental illness with respect for service users’ rights and preferences.
- Use problem-solving techniques to identify and assess need and develop appropriate intervention strategies.
- Assess mental health needs, physical health needs, functional capabilities and care options.
- Ability to make appropriate referrals to community physicians for further assessment and treatment and coordinate psychiatric and medical treatment.
- Offer groups and individual services to educate service users, families and other staff about health and wellness behaviours (e.g., diabetes management).
- Handle ambiguity to function effectively while dealing with competing priorities.
- Familiarity with the Ontario Mental Health Act and Mental Health Reform.
- Knowledge of Recovery as a guiding principle for assessing and developing treatment plans.
- Ability to work with agencies, hospitals and the broader community.
- Knowledge of trends and issues related to the delivery of services to a mental health client population.
- Excellent organizational and interpersonal skills.
- Excellent computer skills (e.g., Word, Excel, Microsoft Outlook).
- Ability to manage and adapt positively to an ever changing health care/work environment.
- Effective communication and written skills.
- Sound clinical knowledge and experience in case management with the targeted population, including individuals experiencing serious mental health issues, addictions, justice system involvement and homelessness.
- Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.
- Visits will be conducted in the service users’ homes, hostels, hospitals and other community settings.
- Weekend rotation: typically 1 in 3, occasionally 1 in 4
- Must have access to vehicle. (Please note that access to a vehicle means owning, borrowing, or renting the vehicle. Alternatively, using ride services, such as, Uber, cab, and taxies at no cost to Cota).
- Visits will be conducted in the service users’ homes, hostels, hospitals and other community settings.
- Weekend rotation: typically 1 in 3, occasionally 1 in 4
