POSITION SUMMARY:


The Outreach Worker provides support to individuals with extensive needs in the home, school, centre and/or, and in the community. Responsibilities include supporting the treatment/program plan(s) and plans collaboratively in partnership with the multidisciplinary team. This worker must be able to work autonomously within a multi-agency team context.


JOB RESPONSIBILITIES may include but are not limited to:

  • Providing goal-based respite support for youth in Hamilton, ages 6 – 17 identified with extensive needs
  • Create and implement Extensive Needs programming
  • To implement individualized goal-focused support in collaboration with Woodview and community partners to provide comprehensive support
  • Communicating and collaborating with families, as well as a variety of professionals and community agencies
  • Monitors client behaviours, identifies concerns/learning needs, and communicates this to Program Manager
  • Participating in ongoing evaluation and appraisal of the effectiveness of all activities and programs
  • Ongoing communication and collaboration with Teammates
  • Flexible schedule including daytime, evening and weekends based on client need and availability
  • Implement client focused activities, responsive to the needs of clients. May be on an individual basis or within a group setting
  • Maintain confidentiality in all aspects of job (written/verbal/implied)
  • Able to work independently as well as collaboratively within an inter-disciplinary team
  • Demonstrate confidence, self-direction and sound judgement in the role
  • Transports individuals or groups of clients in agency vehicles or in their personal vehicles.


Qualifications:

  • Diploma or Degree from a recognized University or College in the fields of Psychology, Autism Spectrum Disorders, Behavioural Sciences, Child & Youth Worker, Developmental Support Worker, Social Service Worker, Autism Behavioral Sciences certificate, or equivalent in education and experience and professional training deemed acceptable to serve the needs of the client population
  • A valid ‘G’ driver’s license and the ability/capacity to drive agency vehicles is required for this position
  • Knowledge of and demonstrated experience implementing strength-based, evidence-based, and collaborative practices and supports with clients and families, and experience in children’s mental health and working with families (minimum 2 years)
  • Sound knowledge and/or experience in the mental health and neurodevelopmental field and an ability to manage the emotional and behavioural needs of clients
  • Strong ability to communicate concise, accurate information in verbal and written reporting
  • Ability to interpret and comply with the policies and procedures of the Agency and Ministry directives with the assistance of the Program Manager
  • Good judgment, initiative, and demonstrated high standard of service delivery and professional skills
  • The ability to work independently and as a supportive and active member of an interdisciplinary team
  • Demonstrated good health. (A certificate indicating the absence of communicable disease is required at the time of employment)
  • The incumbent trained in First Aid/CPR, NVCI, and WHMIS is asset
  • Current police check including a vulnerable sector screen certifying that the applicant has not been convicted of any criminal offense that might be relevant to the requested position
  • ASIST certification an asset
  • Knowledge of DBT, CBT and Applied Behaviour Analysis is an asset
  • Strong organizational, problem solving and decision-making skills
  • French as second language is asset

Effort and Working Conditions:

  • Possesses the physical strength and agility to perform all responsibilities including interacting with individuals in active programs and the use of physical restraint if necessary.

  • Due to the nature of the client population, the incumbent must be able to tolerate situations that can escalate into shouting, aggression, and self-injurious behaviours; and stay calm to implement prescribed behavioral protocols (i.e. redirection, de-escalation, blocking, physical restraints, etc.).

  • Travel will be required, so individuals must be physically capable of operating vehicles safely, possess a valid G driver’s license, and have an acceptable driving record. Access to a vehicle daily is required.

  • The incumbent will interact with clients, family members, staff, and community partners.

  • This position is performed within a variety of milieus including within the community.

  • Work flexible hours and/or shifts as determined by the agency and described in the employment contract, which includes evenings and weekend shifts.
