Your Opportunity:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to clients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of clients and their families. The assignments of the RN in Home Care could include discharge planning, intake, supportive living case management, as well as case management of community clients in a variety of settings. You may be required to cover other assignments as workload dictates under the home care program and will be scheduled to work on call as part of your rotation. You will play a leadership role in supporting an integrated and holistic approach to client care, health promotion and maintenance. You will provide safe, quality client and family centered care while reflecting the shared vision and values of ALA. Reporting to the Care Manager, the RN is responsible for directing the care of clients under the home care program. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning, facilitate client and family teaching. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. Your experience in providing wound care and knowledge of the wound care principles and products, as well as palliative and home parenteral therapy will be essential.

Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

An excellent attendance record, reliability and personal suitability must be demonstrated through your employment references. Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal. Preferred Qualifications:

Minimum of 2 years Home Care experience within the last 5 years of nursing preferred, although, skills and experience relevant to nursing practice in community sector will be considered. Current use of Connect Care, Service Authorizations, RAI-HC, Netcare, and AADLs. Experience with HPT, wound care, client teaching, palliative support. Excellent interpersonal, communication, time management and ability to work in a collaborative team demonstrating adaptability and flexibility.
