Position Summary
St. Joseph’s Healthcare Hamilton’s Renal Transplant Program is one of six adult renal transplant centers in the province. At SJHH, we transplant 120-165 patients annually including living and deceased donor transplants.
The Renal Transplant Clinic focuses on preparing patients with chronic kidney disease for their upcoming kidney transplant surgery. We also work closely with living kidney donors to prepare them for their kidney donation surgery. Once surgery has taken place, transplant patients return to the clinic for follow-up care, management, support, and education. The Renal Transplant Clinic is a dynamic multidisciplinary team consisting of Registered Nurses, Nurse Practitioners, Physicians, Psychologists, Pharmacists, Social Workers, Registered Diabetes Educators, Dietitians, and Administrative Professionals.
Responsibilities

Responsibilities
The Primary Care Nurse is an integral part of the team responsible for the care management of patients donating or receiving kidney transplants. The role of the Primary Care Nurse includes but is not limited to:
- Coordinating the care of patients from the time of referral throughout the transplant journey (encompassing initial assessment, listing, follow-up, donation/transplantation, and post-transplant/donation care)
- Providing patient education and supporting the patients understanding of the transplant journey
- Acting as the key contact person for patients/families, referring physicians, and other healthcare providers both internally and externally
- Collaborating and participating in initiatives locally and with partner organizations such as Trillium Gift of Life Network, Canadian Blood Services, and the Ontario Renal Network
- Current registration with the College of Nurses of Ontario required
- Baccalaureate Degree in Nursing from an accredited University required
- Current BCLS/HCP certification required
- CNeph certification preferred
- 2 years of Transplant nursing experience required, nurses with current Transplant experience will be given preference
- Experience working in an ambulatory care setting preferred
- Current CPI and/or GPA training preferred
- RNAO or other professional association membership preferred
- Advanced case management, organization, time management, and coordination skills with the ability to prioritize workload to meet urgent and rapidly changing timelines.
- Self-directed with strong leadership, problem solving, decision making, critical thinking, team building and interpersonal skills.
- Demonstrated ability to effectively communicate and provide education to patients and families
- Demonstrated ability to practice collaboratively and communicate clearly with members of the interdisciplinary team to provide care for a range of patients/clients.
- Demonstrated proficiency with computers (e.g., Microsoft Office [Word, Outlook, Excel, etc.], internet browser searches, data entry and patient information systems)
- Demonstrated commitment to ongoing learning, professional development and continuous quality improvement
